Job Description FT 37.5 10am-6pm Varied Days including weekends and holidays


Coordinates follow-up care for patients post discharge from the Emergency Department / Inpatient / Outpatient settings. Ensures patients obtain appropriate appointment/ speciality referral based on their care needs.


Job Responsibility


•Responsible for accurately and responsively performing varied adminsitrative tasks and problems of moderate complexity.
•Solves or escalates operational problems and technical issues to senior team members and management; modifies existing tools or processes.
•Coordinate follow-up care for patients based on their referral needs (i.e. specialty care, primary care, preventative care, etc.).

•Utilizes multiple scheduling systems and tools (i.e. EMR, Registration / ADT systems, ListApp, Excel, Soarian, Followmyhealth App, telephone, etc.) to schedule and manage appointments.
•Provide exceptional customer experience for patient, family, visitor, care team members including service recovery as needed.
•Continuously collaborate and build productive provider relationships within the Northwell Network and community practices to enhance patient scheduling needs and programmatic efficiencies.
•Ensures patient is referred to appropriate care provider, based on current insurance plan, patient needs, and diagnosis. Collaborating with insurers, providers and practices, as applicable.
•Round out department to proactively identify patients that require additional scheduling support and serve as a care navigator specifically related to social determinants of health.
•Optimize the discharge experience focused on a seamless referral process for the patient.
•Provide referral coordination as a service to employees as requested/ if applicable.
•Collects, organizes, and analyzes data to understand program success and opportunities for improvement, as applicable.
•Participate in departmental work groups and patient care improvement initiatives, as required.

Job Qualification


•High School Diploma or equivalent, required.
•Associate's Degree or equivalent combination of education and related experience, preferred.
•1-3 years of administrative experience, required and in a health care setting,
•Working knowledge of PC applications, required.
