Medical Guardian is a fast-paced and rapidly growing medical alert and remote patient monitoring company with a substantial Health Services division. We are seeking a Wellness Advocate to join our team. The Wellness Advocate will support whole-person and preventive care for patients and members we serve across the country. The right candidate for this role will be caring, empathetic, and have an unmatched passion for creating positive patient/member experiences. Our Wellness Advocates will work directly with patients/members and caregivers to guide them through ways to use their wearable medical alert device to support health and wellness, along with providing personalized support related to their individual care needs. Medical Guardian helps health plans and providers close gaps in care to improve outcomes. This role combines the use of technology with human interaction to provide a customized health services offering that allows patients/members to age in place, helps them to overcome challenges and takes their care monitoring to the next level.

Residency Requirement:

All applicants are required to have permanent residency in one of the following states: AZ, NV, DE, FL, GA, KY, MA, MD, MI, NJ, NY, OH, PA, SC, ID, IL, NC, CO, WV, and TX only

Responsibilities:

  • Receive inbound phone calls from members and caregivers in response to various wellness outreach campaigns.
  • Complete outreach efforts to provide ongoing support and coordination of individual health and wellness services such as routine healthcare services (e.g. PCP appointments) and preventive screenings (e.g. flu shots and cancer screenings).
  • Follow up on unresolved service and outreach requests.
  • Educate members on and promote usage of services and resources available through partner programs.
  • Communicate updates with agencies on patient/member's behalf as needed per established procedures.
  • Maintain superior knowledge of the Medical Guardian suite of products and services to ensure that information communicated to the member is accurate and complete.
  • Guide callers through troubleshooting and effectively using their Medical Guardian products and services.
  • De-escalate situations where needed, research issues and offer solutions to resolve problems.
  • Maintain professionalism, interact positively with others and create and nurture effective working relationships.
  • Ensure confidentiality of Protected Health Information (PHI)
  • Adhere to all Company procedures, protocols, processes.
  • Meet or exceed the established productivity and quality goals.
  • Other duties and responsibilities as assigned.

Required Qualifications:

  • 3-5 years of experience in a role with Health or Human Services background.
  • Effective active listening skills with an ability to assess situations to provide appropriate action and resolution.
  • Ability to work both as part of a team and independently
  • Bachelor's degree or equivalent work experience. Preferably in Social Services or Healthcare Administration
  • Proficiency in Microsoft Office suite of applications
  • Excellent written and verbal communication skills

Preferred Qualifications:

  • Experience with Medicaid, Medicare Advantage health plans, or Home Care
  • Experience with Salesforce
  • Familiarity with healthcare regulations and compliance
