The Pharmacy Care Coordinator is a pivotal member of the Optum Clinical Excellence Team. This role actively engages with patients telephonically, Landmark providers in person, and community providers, pharmacies, insurance companies telephonically. The incumbent will use compassion and empathy to earn patients' and caregivers' trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships.

Hours: Monday - Friday 8am-5pm CST

Primary Responsibilities:

  • Consistently exhibits behavior and communication skills that demonstrate Optum's commitment to superior customer service, including quality, care and concern with each and every internal and external customer
  • Documents pertinent patient information in the electronic medical record
  • Assists with data management and recording results for month-end reports
  • Assists in Pharmacy Services wellness programs (i.e. transition of care medication reconciliation) by:
    • Retrieving pertinent patient information for new patient referrals or recent hospital/SNF discharge
    • Contacting patients post hospitalization to perform medication review and information gathering
    • Performing patient follow-up procedures
    • Answering incoming phone calls and directing patient messages
    • Coordinating with other healthcare providers, healthcare professionals, and healthcare team members
  • Updates job knowledge by participating in educational opportunities and reading professional publications
  • Participates in process improvement initiatives, staff meetings and departmental meetings
  • Uses, protects, and discloses Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Adheres to Universal Precautions and safety policies and procedures at all times
  • Other duties as assigned

Required Qualifications:

  • High School Diploma /GED (or higher)
  • Possess and maintain an active Pharmacy Technician license in the state of Washington
  • 1+ years of experience as a Pharmacy Technician
  • 1+ years of experience in customer service
  • 1+ years of experience with medical and pharmacy terminology
  • 1+ years of experience with medication therapeutic categories
  • 1+ years of experience with identifying generic name for brand name drugs and vice versa
  • 1+ years of experience with an understanding of indication for medication
  • Basic level of proficiency in Windows and MS applications (Word, Excel)

Preferred Qualifications:

  • National Pharmacy Technician Certification
  • Possess and maintain an active pharmacy technician license in the state of Washington
  • 3+ years of experience working as a pharmacy technician
  • 1+ years of experience in hospital, ambulatory care or other healthcare setting

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.74 to $31.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
