Patient Care Coordinator - Home Health Admin

The Patient Care Coordinator reviews and processes paperwork turned in by field clinicians for accuracy, prepares physician orders/plan of treatments for signature from physician, daily tasks of uploading of documentation into patients EMR. Assists with supplies for clinicians for the patients. Assists with faxing, phone calls, preparing admission packets for our field clinicians. Maintains all physician orders, referral information, and service notes from all disciplines, lab reports, and other clinical documentation in an organized manner. Assists with managing and ordering of the home health nursing supplies, maintaining supply room with adequate supplies and sufficient par levels.

Location: 19500 West Interstate 10, Building 2 - Floor 3 San Antonio, TX

Schedule: Monday to Friday 8:00am to 5:00pm and 9:00am to 6:00pm, rotating schedule. Mandatory overtime when business requires.

Primary Responsibilities:

  • Reviews and processes paperwork turned in by field clinicians for accuracy
  • Prepares all physician orders and plan of treatments for the medical liaison to return to the physician's office for signature
  • Prepares outstanding list of physician orders/plans of care for manager on weekly basis
  • Assists with follow-up on orders/plans of care not returned within 30 days
  • Reviews and clears out print que daily
  • Scans documentation into clinical records (EMR - WellSky) daily
  • Faxes all communication notes, missed visits, and case conferences to physicians daily
  • Contacts field clinicians for corrections on paperwork submitted, as needed
  • Maintains all physician orders, referral information, and service notes from all disciplines, lab reports, and other clinical documentation in an organized manner
  • Reviews therapy contract invoices to ensure payment accuracy
  • Assists with phone calls in office
  • Communicates messages for Clinical Team Manager (CTM) regarding physician orders and patients' concerns
  • Reviews outstanding documentation lists daily, works with CTMs on current lists for completion from field clinicians
  • Assists with managing and ordering of the home health nursing supplies, maintaining supply room with adequate supplies and sufficient par levels
  • Performs additional duties as assigned by manager

Required Qualifications:

  • High School Diploma/GED (or higher)
  • 1+ years of experience in the medical field
  • Intermediate level of computer proficiency with word processing software
  • Intermediate level of proficiency operating computer terminals, printers, faxes, and office copier machines
  • Ability to type 40 WPM
  • Ability to lift at least 25 lbs

Preferred Qualifications:

  • Working knowledge of medical terminology
  • Knowledge of general office procedures

Work Environment:

  • Performs duties in the office environment during Agency operating hours. At times, noise levels may be moderately high
  • Requires ability to handle stressful situations with calmness and courtesy at all times
  • Occasionally, may need to work extended hours
  • Acceptable footwear for supply rooms: closed toe shoes with rubber/slip resistant sole with a heel not greater than one inch in height must be worn for safety
  • Ability to maintain flexibility and adaptability to the work environment is essential
