The Field Care Coordinator will be the primary care manager for a panel of members with low-to-medium complexity medical/behavioral needs. Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.
This is a field-based role that will require travel approximately 25 -50% of the time in the field within an assigned coverage area. Candidates must be willing to travel throughout the Frederick County, Fauquier County, Clarke County and Winchester, VA Markets.
If you are located in Frederick County or surrounding areas, you will have the flexibility to work remotely as you take on some tough challenges. This is a field based role with a home-based office.

Required Qualifications:
- Bachelor's degree in a health or human services field
- 1+ years of care coordination or behavioral health experience and/or work in a healthcare environment
- 1+ years of experience with MS Office, including Word, Excel, and Outlook
- Experience working with members who have medical needs, the elderly, individuals with physical disabilities and/or those who may have communication barriers
- Access to reliable transportation and the ability to travel within assigned territory to meet with members and providers
- Reside within a commutable distance of Frederick, Fauquier, Clarke or Winchester Counties
Preferred Qualifications:
- CCM certification
- Experience working with Medicaid/Medicare population
- Long term care/geriatric experience
- Experience working in team-based care
- Demonstrated background in Managed Care
