Job Description
The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivated and talented individual for the Human Services Coordinator I (HSC I) position in the Southeast Region. The Human Service Coordinator I is the primary contact for individuals and families served by the Department and are expected to effectively represent the Area Office by building respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Area Office and Department.
The HSC I coordinates and facilitates the development of Individual Service Plan (ISP) for individuals on assigned caseload, as well as monitors the implementation of services provided by DDS funded residential, day and support program providers. The HSC I ensures individuals on caseload receive appropriate services through arranging, organizing and coordinating the Department of Developmental Services funded and generic support services in response to individual's needs, while maintaining communication and outreach with community services and other state agencies.

Additional HSC I responsibilities include: monitoring the safety and well-being of assigned individuals; providing advocacy in human, civil and legal rights; facilitating team meetings; monitoring follow-through on team recommendations while maintaining individual's records and relevant computer databases, as well as maintain regular and consistent contact with individuals, service providers and families. The HSC I will arrange or provide crisis intervention for individuals as needed, assessing needs and making recommendations for follow up planning, as well as providing information to supervisor and other members of the Area Office Management Team, as needed.
Duties and Responsibilities
• Coordinates and facilitates the development of Individual Service Plans (ISP) for assigned individuals in accordance with established DDS policies, procedures and regulations.
• Identifies and contacts participants for ISP meetings, schedules meetings, coordinates assessments, facilitates the Team's input and the development of the ISP.
• Accurately enters all pertinent ISP data into the electronic file per DDS guidelines and within established timeframes,
• Coordinates services and communication with other state agencies, service providers, medical providers, other clinical professionals, and involved family members to coordinate the delivery of needed services.
• Advocates for needed services; maintains communication with Area Office supervisor.
• Monitors services and convenes the Team if modifications to the service plan are needed.
• Completes assigned obligations of the Medicaid Waiver program; documentation such as the Plan of Care, Level of Care, and
• Depending on assignment, assists individuals with self directed services.
• Assists and transports individuals to appointments and meetings.
• Coordinates and arranges for services and supports for individuals currently without an ISP which may include safe housing, training, employment, day programs, and specialized or generic support services such as transportation, respite care, family support, medical services and document in accordance with DDS rules and regulations, l l . Consults with individuals, families, guardians, and acts as a liaison within DDS and with other state agencies, service providers, generic medical service providers, private vendors, and clinical professionals to coordinate the delivery of services and supports.
• Communicates professionally and respectfully with all parties at all times.
• Monitors the implementation of the ISP for each individual through on-site program visits according to DDS policy.
• Coordinates referrals and assists individuals with procedures to assess finances, eligibility for SSI, SSA, affordable housing and other forms of public assistance,
• Maintains the individual's written and electronic records to insure confidentiality, completeness, accuracy, and compliance with
• established timelines and DDS policies.
• Completes site visits in accordance with Site Visit Policy.
• Completes progress notes and billing as required.
• Provides information to the Area Office administration regarding services and statuses which may include but is not limited to Meditech, RICCI Court Reporting forms, Transportation Authorization Forms, Respite Care Service Requests, HCSIS Incident reporting, Health Care Record, Death Reporting.
• Assists in the identification and arrangement for emergency and/or crisis intervention services.
• Participates in forums to address risk management concerns and completes necessary repofis and follow-up as needed.
• Other related duties.
Preferred Qualifications
• Knowledge of the principles and theories of individual and family growth and development.
• Knowledge of the types and manifestations of developmental, psychological and emotional disorders.
• Knowledge of interviewing techniques.
• Ability to communicate effectively through verbal and written means.
• Sensitivity to religious, ethnic and linguistic diversity of individuals with mental retardation/developmental disabilities and their family members.
• Ability to evaluate and maintain accurate records.
• Ability to deal tactfully with others, and to establish and maintain harmonious working relationships with others.
• Ability to demonstrate skill in handling confidential information.
• Ability to prioritize, responds to multiple demands, and manages time efficiently.
• Ability to seek guidance and support and take direction from supervisors and clinical consultants when needed.
Schedule
Hours: 37.5- full time position.
Shift: 1st shift
Days Off: Saturday and Sunday
Flexibility required to meet the needs of the Area Office.

Qualifications

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.
Substitutions:
I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.
Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range .
