Care Manager/Social Worker

Position Summary: Care Managers bridge the gap between health systems and community-based aging services. Care Managers work with health systems and providers to identify and serve older adults and their informal caregivers that would benefit from programs and opportunities that are available within the community. Care Managers offer a holistic assessment of the client's needs by identifying health, financial, emotional, functional and environmental concerns. Care Managers offer care options, solutions and guidance maneuvering through challenging systems.

Principal Responsibilities:

  • Provides care coordination navigation for referred clients including:

o Accepts referrals from medical practice, home care agency or other health system provider.

o Completes comprehensive in-home assessments to determine needs.

o Creates and implements care plans that meet clients' needs and preferences.

o Maintains close communication with client's primary care physician to provide informed care.

o Monitors care plans client needs, reassessing and revising care plans as needed.

o Provides information and referral, advocacy, benefit, and entitlement counseling.

  • Arranges mental health interventions for older adults and their caregivers, when necessary. This includes:

o Conducts depression and anxiety screenings, when appropriate.

o Screens for caregiver stress, when applicable.

o Provides supportive counseling to older adults and their caregivers.

  • Participates as a member of the inter-agency team:

o Participates in all mandatory staff meetings and trainings

o Participates in all applicable primary care office/health system interdisciplinary meetings.

o Participates in Quality Assurance activities.

o Accepts and seeks guidance and clinical supervision.

  • Maintains up to date professional knowledge of the geriatric field, community services entitlement benefits, clinical skills and competencies through reading, workshops and conferences.
  • Being aware of and actively supports the Agency's initiative of being collaborative and community focused, innovative and culturally competent.
  • Other duties as assigned by supervisor.
  • Must maintains up to date professional knowledge of the geriatric field, community services entitlement benefits, clinical skills and competencies
  • Adhere to organization's standards by completing annual mandatory trainings in a timely manner on topics including - but not limited to - Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regard to regulatory compliance or good business practices.
  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers.
  • Adheres to the organization's policy regarding absenteeism and appearance and health and safety standards.

Compensation : $58,000 - $61,800 commensurate with experience.

Job Competencies & Minimum Qualifications:

  • MSW preferred. Will consider bachelor's degree in similar field
  • Minimum of two-years of experience in social work and at least one year of experience in geriatrics.
  • Must be able to conduct in-home-visits.
  • Working knowledge of Microsoft Office Suite and other technology
  • Excellent communication and listening skills.
  • Excellent customer service skills with a focus on treating clients with respect and dignity.
  • Able to multi-task and work independently with a great attention to detail

Working Conditions/Physical Demand:

  • Business office environment with phone and computer use.
  • Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
  • Employee will be expected to travel to client's home via public transportation.
  • This position is hybrid schedule with office/field work 2-3 times per week.

Bachelors or better in Other or related field

Masters or better in Social Work or related field
