Physical Therapist

Description

The Home-Based Acute Care Physical Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home setting in accordance with established policies while maintaining the highest degree of quality patient care. As a member of the multi-disciplinary team the therapist assumes responsibility for communication of patient-related issues with appropriate team members or other staff and participates in collection and evaluation of data necessary to assess appropriateness of care at home. The Physical Therapist may also be assigned to orient staff and students. The Physical Therapist works with the patient and family/caregivers in the home and hospital setting without direct supervision.

Required Qualifications:

  • Education sufficient for licensure
  • Upon hire: Washington Physical Therapist License Or
  • Upon hire: Washington Physical Therapist Temporary License Or
  • Upon hire: Washington Physical Therapist Interim Permit
  • Within 30 days of hire: National Provider BLS - American Heart Association
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 1 year Therapy experience with adult geriatric and/or individuals with physical disabilities

Preferred Qualifications:

  • 1 year Therapy experience in home care or in related rehabilitation setting.
  • Experience with the geriatric population.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
