Job Description
. Outpatient: • Scheduling and maintaining daily schedule. • Physician initial order or referral. • Diagnosis for therapy services including date of onset; initial evaluations and all re-evaluation in accordance to Medicare and Insurance guidelines. • Plan of treatments with MD signature pertaining to claim period. • Progress notes and attendance records pertaining to the claim period to support total minutes of actual treatments and number of minutes for each modality • Clinical progress notes. • Perform videlfluroscopic swallowing studies. 2. Hospital: • Patient education sheet. • Multidisciplinary care plan. • Education for nursing staff and PCTs. • Updated forms and policy. • Perform standardized patient testing on each patient when appropriates. • Perform videofluoroscopic swallowing studies.

Job Qualification
• Master's Degree in Speech Pathology, required. • Currently licensed in Speech Pathology in the State of New York. • Certificate of Clinical Competency from the American Speech-Language-Hearing Association, required. • Eligibility for certification in Speech Pathology by American Speech and Hearing Association.
