DaVita is looking for a Part Time Licensed Social Worker (LISW) (LICSW) (LGSW) to support our patients and their families at our East River Road and River City Dialysis Clinics in the greater Minneapolis, MN area.

• Part time schedule 24-30 hours
• Training - 6-8 weeks
• Coverage will include East River Road (Fridley, MN 55421) and River City (Stillwater, MN 55082) dialysis clinics
• Will provide group supervision 4 hours a month that is no cost to the teammate

• Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Requirements:

• Licensed or registered in the state of practice as required by state regulations -- (LISW) (LICSW) (LGSW)
• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
