- After training is complete and you feel comfortable in the role, this Social Worker may float to other clinics.
- Schedule: Work 3 days a week, normal business hours.
- We offer paid mileage and ability to autonomously manage your own schedule that works for our patient needs.

- Conducts psychosocial related assessments and provides recommendations for patients. May require guidance or assistance.
- Completes CMS mandated and social work specific tasks.
- Independently completes initial patient psychosocial assessment within 30 days of admission, and conducts an independent reassessment within 90 days after the initial assessment.
- Identifies and develops the treatment and psychosocial goals for an initial plan of care and updates the plan of care as outlined by the clinic's governing body for stable and unstable patients.
- Identifies, assesses, and documents psychosocial evaluations as required by CMS, state and/or DaVita P&P.
- Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing.
- Conducts, reviews, and assesses quality of life survey results, develops patient goal setting, completes appropriate follow up to analyze patients' plan to improve scores and quality of life. To be performed periodically -- annually at minimum
- Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing
- Counsels new and current patients and families on treatment options and modalities (e.g. transplantation, PD); provides support, information, and assistance in treatment planning; provides assistance to patient and family in adjusting to dialysis and altered lifestyle; provides resource information.
- Conceptualizes problems, interventions and the evaluation of interventions with respect to human behavior, personality development, and social environment. May require supervision and/or assistance as new to role.
- Complies with state and federal mandatory reporting laws such as Adult Protective Services and Child Protective Services.
- Partners with other MSWs to analyze information and facilitate referrals to community, private, state, and federal agencies based on psychosocial needs (e.g. financial programs, medication assistance programs, Vocational Rehabilitation Programs, transportation resources, mental health and chemical dependency resources, and home health/homemaking agencies).
- Basic knowledge of government and private insurance programs as well as Nephrology social work.
- May consult Intermediate, Experienced, Advanced, or Expert MSWs for expanded scope of practice.
- Master's Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
- Meets all state required regulations to practice in the dialysis setting.
- Obtained their Clinical License (e.g., LCSW, LICSW), meeting all licensure requirements for their state.
- Prefer some renal experience or exposure to ESRD patients, but not required.
- Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, as well as functional proficiency with DaVita specific applications, within 60 days of employment.
- Demonstrated knowledge of government and private insurance programs.
- Excellent interpersonal and communication skills
