DaVita is looking for a Full-Time Nurse Practitioner (NP) to join us in Beaumont, TX region (covering Orange, Port Arthur, Nederland, TX with occasional travel to Houston, TX).
As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You'll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients. You'll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.
Location: Round and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.

Position Details:
- Complete Comprehensive Health Assessments (80%) of the time
- Monday- Friday schedule with the ability to accommodate patient and nephrology partner's availability. Must be flexible.
- Holistic management of patient conditions, including assessment, diagnosis, treatment, and consultation.

About You:

- Minimum of two (2) years' experience as a Nurse Practitioner (NP) working in Value Based Care including documenting and coding from an MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
- Current unrestricted NP license to practice in the state
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Passion for delivering care to patients with complex, chronic illnesses
- Home Office, High Speed Internet and Reliable Transportation Required.
- Experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
- Spanish-speaking preferred in some markets
- Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)
- Manage pop. Health (hospitalization)

Salary/ Wage Range
$115,000.00 - $140,000.00 / year
