It takes a Village to care for our patients and as a Care Coordinator (CC), you will be at the heart of all the action. Our CC's work together as a team to facilitate the planning and organizing of all critical activities for our POST TRANSPLANT patients. The patients we care for are some of the most complex and need an excellent care team to help manage their conditions. The Care Coordinator will work closely with our clinical teams to make sure our patients receive the highest quality care.

- Position is fully remote, must reside in Georgia. Atlanta Metro Area preferred
- Monday through Friday schedule
- Full time, 40 hours per week

Patient Engagement & Navigation
- Conduct initial outreach to new patients, gather intake information, and schedule consultations and evaluations
- Provide patients with clear, accurate education regarding the transplant process, required testing, timelines, and next steps
- Serve as a primary point of contact for patient questions, follow-ups, and care coordination needs
- Own and manage the patient call center function, ensuring timely, empathetic, and effective communication
- Track and respond to patient inquiries, coordinate messages between patients and providers, and document communication in the EHR
- Coordinate multidisciplinary care plans and logistics across departments
- Collaborate with Nephrologists to review and support patient care plans
- Ensure accurate sharing of clinical documentation, test results, and care notes across teams
- Track referral status and patient readiness in coordination with clinical leadership
- Assist the Administrator in processing and tracking incoming referrals
- Ensure all required records, labs, imaging, and documentation are collected for each referral
- Monitor and maintain patient tracking systems, performance metrics, and status updates
- High school diploma or equivalent required; associate's or bachelor's degree preferred
- Minimum of 2 years of experience in a healthcare administrative or patient-facing role
- Experience in nephrology, transplant services, or specialty care preferred
- Strong communication and interpersonal skills, with the ability to support patients through complex medical processes
- Proficiency in EHR systems (e.g., Epic) and Microsoft Office Suite
- Excellent organizational skills, with the ability to prioritize, multitask, and maintain attention to detail

Salary/ Wage Range
$19.00 - $27.00 / hour
If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
• New York Exempt:
• New York City and Long Island: $64,350.00/year
• Nassau, Suffolk, and Westchester counties: $64,350.00/year
• Remainder of New York state: $60,405.80/year
New York Non-exempt:
• New York City and Long Island: $16.50/hour
• Nassau, Suffolk, and Westchester counties: $16.50/hour
• Remainder of New York state: $15.50/hour
Washington Exempt: $77,968.80/year
Washington Non-exempt:
• Bellingham: $17.66/hour
• Burien: $21.16/hour
• Unincorporated King County: $20.29/hour
• Renton: $20.90/hour
• Seattle: $20.76/hour
• Tukwila: $21.10/hour
• Remainder of Washington state: $16.66/hour
