MTC Pharmacist - Spanish Bilingual

The role of the Spanish-speaking MTM Pharmacist is to deliver telephonic medication therapy management (MTM) for our Spanish population. Effective delivery of this CVS Health telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers. The MTM Pharmacist interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health related goals and help them on their path to better health. Pharmacists will also be trained and use the innovative skill of behavioral modification and interviewing, which are key drivers to positively impact patient health outcomes.Required Qualifications
Active pharmacist license
Bachelor's Degree in Pharmacy or PharmD from an accredited school of Pharmacy
At least 4 months current experience within the Medication Therapy Counseling Department.
Bilingual fluency in Spanish and English
Preferred Qualifications
Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
Basic computer skills and proficiency with Microsoft Office products.
Strong organizational skill, interpersonal skills, and detail orientated.
Experience maintaining focus and meeting goals in a work from home environment.
Previous call center experience
Education
Bachelor's degree in pharmacy or Pharm D.
Must have an active pharmacist license in good standing. Anticipated Weekly Hours

The typical pay range for this role is:$50.48 - $84.62This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
