Are you a Licensed Social Worker with a passion for behavioral health and community-based care? Join our mission-driven team as a Field Behavioral Health Case Manager, where you'll play a vital role in supporting members with complex needs through compassionate, coordinated care.
What You'll Do:
As a Behavioral Health Case Manager, you will:
- Conduct in-home, face-to-face assessments for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Programs (D-SNP/FIDE).
- Evaluate behavioral, physical, and psychosocial needs to develop individualized care plans.
- Coordinate services such as adult/pediatric medical daycare, personal care assistance, and nursing facility placements.
- Collaborate with members, families, providers, and interdisciplinary teams to ensure appropriate care and community resource access.
- Advocate for members during transitions of care and attend interdisciplinary meetings.
- Accurately document assessments and care plans in the electronic health record.
- Mentor new hires once proficient in your role.
Required:
- Must reside in Passaic County, NJ
- Active Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) in New Jersey
- 2+ years of behavioral health experience
- 1+ year of experience using computers and Microsoft Office
- Reliable transportation and willingness to travel 50-75% within Passaic County and surrounding areas
- Successful completion of NJ Choice Certification (80%+ exam score, training modules, and field mentoring)
- Experience in case management or discharge planning
- Background in Managed Care
- Strong crisis intervention skills
- Master's Degree in Social Work or a Behavioral Health-related field (required)

The typical pay range for this role is:$72,627.00 - $155,538.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
