Position Summary:

Oversees and participates in providing quality & timely access services to prospective and new patients and physicians referring patients to City of Hope. Assures timely coordination of obtaining medical information and coordination of appointment. Performs all duties within the scope of certification utilizing the nursing process within a team approach to total patient focused care. Acts as a patient advocate in all ethical and legal aspects.

Key Responsibilities:
  • Oversees all aspects of new patient access for their assigned service. Delegates various assignments to support staff based on patient acuity and staff availability. May refer more clinically complex cases to the NPS Clinical Nurse III.
  • Conducts new patient assessments utilizing the following parameters: clinical history, current performance status, financial and insurance profile, social concerns. Clinical history and current performance status are documented at point of service in the patient EMR
  • Triages telephone calls appropriately and expeditiously. Investigates and/or ensures that questions and requests for information are responded to in a timely and professional manner.
  • Ensures patient have timely access to care by facilitating a warm hand off / one call resolution with the patient receiving an appointment date and time at the end of the call
  • Ensures new patient physician appointment are schedule at point of service for case where physician review is not require.
  • Ensures proper and timely documentation in the patient's EMR and patient's account in Salesforce, including case disposition
  • Prior to initial new patient appointment ensures all outside medical studies are received and are current in EMR
  • Oversees and assists other staff members and/or disciplines, including prioritizing and resolving questions.
  • Utilizes leadership skills in overseeing and assisting members of the health care team to prioritize and resolve issues.
  • Maintains an appropriate level of productivity including meeting department's warm hand off metrics
  • Maintains appropriate level of accuracy for work performed
  • Demonstrates a level of professional practice that supports the delivery of appropriate care and positive working relationships.
  • Maintains a safe, comfortable and therapeutic environment for patients/families in accordance with hospital standards.
  • Enhances professional growth and development through participation in educational programs, reviewing current literature, attending in-service meetings, and workshops.
  • Maintains awareness of ongoing performance improvement studies within the unit; assists with the implementation of continuous improvement and solutions determined necessary.
  • Maintains departmental records and files for efficient operations. Assist with identification of trends and issues as it relates to marketing, public announcements, and factors affecting Patient Access
  • Attends and actively participates in meetings as required. Performs other related duties as assigned or requested.
Basic Education, Experience and skills required for consideration:
  • Completion of a licensed vocational nursing program
  • 0-1 year of work experience as an LVN in a medical environment.

Required Certification/Licensure:
  • Current LVN License in the state of California.

Preferred Education Experience and skills:
  • Data entry & computer experience and knowledge of managed care reimbursement factors preferred

Additional Information:
  • This position is represented by a collective bargaining agreement
  • To protect the health of patients and staff and to comply with new State of California mandates, City of Hope staff are required to show proof of full vaccination by September 30, 2021. Compliance is a condition of employment.
