Position Summary

Designs and implements appropriate plan of nutritional care for patients at nutrition risk. Provides on-going nutritional care and monitors patient progress toward nutrition goals. Provides nutrition counseling and follow-up. Serves as a resource and provides educational sessions to staff, other health care professionals and the community on current nutrition topics. He or she is responsible for assessing educational needs of patients/staff; planning and implementing staff development activities; evaluating the effectiveness of those activities for patient/ staff.

"Certified Diabetes Care and Education Specialist or Certified Diabetes Educator® (CDCES or CDE ®) is a health professional who possesses comprehensive knowledge of and experience in diabetes prevention, prediabetes, and diabetes management. The CDCES or CDE ® educates, supports, and advocates for people affected by diabetes, addressing the stages of diabetes throughout the lifespan. The CDCES or CDE ® promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes". Defined by the Certification Board for Diabetes Care and Education.

Key Responsibilities include:
  • Provides services that are accurate, reliable, courteous and timely.
  • Delivers care that is consistent with professional practice standards and the mission and values of City of Hope.
  • Demonstrates competence appropriate to their role and function in age-specific care and nutrition management and diabetes self-management education.
  • Delivers quality care consistent with the scope of practice of the discipline.
  • Provides timely, comprehensive and complete education/teaching to patients and families and documentation of progress towards achievement of outcomes.
  • Delivers care for a defined group of patients using effective delegation, supervision and decision-making skills.
  • Provides timely, evidence-based, and appropriate education to medical staff, other health care professionals and the community.
  • Ensures that all professional and regulatory standards are met and maintained.
  • Effectively communicates with all customers/team members to meet organizational goals and customer service expectations.
  • Maintains current knowledge, skills and competencies through involvement in professional continuing education activities.
  • Promotes the efficient and effective utilization of resources.
  • Demonstrates performance improvement activities to promote continuous improvement in care delivery.
  • Demonstrates assimilation of core organizational values within own performance.
  • May participate in research activities related to the field of nutrition and diabetes.
  • May assist in the preparation of manuals, guides and other materials contributing to improving clinical performance, updating staff instruction and the maintenance of patient care services.
  • Conducts well planned, organized, relevant and clinically oriented programs in response to needs.
  • Coordinates continuing education workshops.
  • May plan orientation program to meet individual learning needs: develops objectives for each instructional program.
Basic education, experience and skills required for consideration:

Minimum Education:
  • Bachelor's Degree with four years recent experience or Master's degree with two years of experience as a clinical dietitian.
Minimum Experience:
  • 3+ years recent experience as a clinical dietitian.
Req. Certification/Licensure:
  • Registered Dietitian;
  • For Endocrinology position, Certification as a Diabetes Educator
  • For pediatric service, CCS Panel provider or complete application within 60 days of hire
Preferred Education:
  • Nutrition.
Pref. Certification/Licensure:
  • Board Certification as a Specialist in Oncology Nutrition, Diabetes Educator or Nutrition Support Clinician."
