We are hiring a Disability Case Manager - Virtual - Canada (Bilingual Preferred)

Reporting to the Manager, Disability Operations, you are responsible for assisting employees on behalf of the employer when the employee is off work or at risk of being off work for health reasons. The case manager assesses any barriers to a successful and timely return-to-work and engages with stakeholders to implement strategies to address those identified barriers and support return to work or an appropriate alternative plan.

Essential Functions

  • Conduct case assessments, contacting the employee and their direct manager by telephone to explain the disability management process and gather information for assessment purposes.
  • Develop and document an understanding of the situation and the factors supporting as well as inhibiting a successful return to work for the employee.
  • Provide any of the following interventions: more in-depth assessment, medical management interventions, initiation, and coordination of occupational and/or psychological interventions, utilization of specialized resources or consultations etc.
  • Determine an appropriate RTW goal with the employee and the employer, provide active support for the return-to work planning process between the employee and their supervisor and intervene when necessary.
  • Communicate proactively with the employer's HR representative regarding any case management issues that may influence the workplace and RTW planning.
  • Manage the transition to LTD and provide medical and summary case management information to the LTD insurer to ensure a seamless transition.
  • Attend Case Management and Disability Management department team meetings.
  • Other projects and tasks as assigned.

Qualifications

  • The ideal candidate has a minimum 5 years of Disability Case Management experience or equivalent combination of experience and specialized education.
  • University degree in a related field. Additional course work in occupational health, rehabilitation, legislation related to disability management, helping relationship skills, mediation, human resource management, disability insurance, psychological and workplace factors in disability would be an asset.
  • You have strong interpersonal and helping skills along with superior assessment and problem-solving skills.
  • French/English language skills are an asset.
  • Demonstrated ability to manage complex tasks under tight deadlines is a core competency to be successful in the role.
  • We are a customer service focused delivery environment where flexible working hours may be required to meet deadlines.

Relevant du gestionnaire des opérations d'invalidité, le gestionnaire de cas d'invalidité est chargé de fournir au nom de l'employeur une assistance aux employés qui sont en arrêt de travail ou qui risquent de l'être pour des raisons de santé. Le rôle consiste à évaluer les obstacles à un retour au travail harmonieux et en temps voulu et à s'engager avec les parties prenantes à mettre en œuvre des stratégies pour éliminer les obstacles identifiés et soutenir le retour au travail ou un autre plan d'action approprié.

Principales responsabilités

  • Effectuer des évaluations de cas, en contactant l'employé et son supérieur direct par téléphone pour expliquer le processus de gestion de l'invalidité et recueillir les informations nécessaires à l'évaluation.
  • Développer une compréhension de la situation et des facteurs favorables ou défavorables à un retour au travail harmonieux pour l'employé. Documenter cette situation et ces facteurs ainsi que leur analyse.
  • Fournir les interventions suivantes, selon le cas : évaluation plus approfondie, gestion d'interventions médicales, initiation et coordination d'interventions professionnelles ou psychologiques, utilisation de ressources ou de consultations spécialisées, etc.
  • Déterminer un objectif de retour au travail approprié avec l'employé et l'employeur, apporter son soutien de façon active au processus de planification du retour au travail entre l'employé et son superviseur et intervenir si nécessaire.
  • Communiquer de manière proactive avec le représentant des RH de l'employeur sur toute question de gestion de cas pouvant influencer le lieu de travail et la planification du retour au travail.
  • Gérer la transition vers l'ILD et fournir des informations médicales et des informations sommaires sur la gestion des cas à l'assureur de l'invalidité de longue durée afin d'assurer une transition harmonieuse.
  • Participer aux réunions d'équipe des services de gestion des cas et de gestion de l'invalidité.
  • Sur demande, effectuer d'autres tâches ou participer à d'autres projets.

Compétences

  • Le ou la candidat(e) idéal(e) possède au moins 5 ans d'expérience en gestion de cas d'invalidité ou une combinaison équivalente d'expérience et d'éducation spécialisée.
  • Diplôme universitaire dans un domaine connexe. Une formation supplémentaire sur la santé au travail, la réadaptation, la législation relative à la gestion de l'invalidité, les compétences en matière de relations d'aide, la médiation, la gestion des ressources humaines, l'assurance invalidité ou les facteurs psychologiques et le milieu de travail dans l'invalidité est un atout.
  • Fortes aptitudes pour les relations interpersonnelles, les relations d'aide et un esprit d'analyse axé sur la résolution de problèmes.
  • Les compétences linguistiques en français/anglais sont un atout.
  • Une capacité démontrée à gérer des tâches complexes dans des délais serrés est une compétence essentielle pour bien assumer ce rôle.
