Develops, facilitates, and/or coordinates stroke patient navigation services, functioning as a patient advocate and resource, initiative-taking case coordinator, and clinical educator for patients, caregivers and providers.

- Coordinates, monitors, and facilitates key aspects of stroke patient care, working collaboratively with multidisciplinary clinical staff.
- Provides consultation to staff, participates in patient rounds and case conferences, and provides community education as indicated.
- Works collaboratively with Stroke Program Leadership, nursing leadership, medical staff, and others to design and implement targeted programs and initiatives.
- Analyzes Stroke Patient Navigator data and provides report of findings to Stroke Steering Committee with the support of Stroke Manager.
- Assists in developing and conducting quality improvement studies.
- Registers stroke patients into the Stroke Navigation Program
- Interviews, assesses stroke patients/caregivers to identify risk factors, barriers and need for community resources.
- Follows up with discharged stroke patients to review medication(s) list and physical therapy ability to obtain medications, review and educate patients/caregivers of their individual risk factors and stroke prevention, assess physical therapy outcomes, refers patients to Stroke support group.
- Provides guidance for stroke patients, families/ caregivers through healthcare continuum.
- Identifies patients’ needs and escalates to multidisciplinary teams.
- Performs other duties as assigned.
MINIMUM QUALIFICATIONS
Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Required Education: Bachelor’s degree in Nursing (BSN) or graduate of an accredited Nursing Program.
Preferred Education: Master’s degree in nursing (MSN).
Required Experience: Five years acute care nursing in Neuro/Tele unit, ICU or ED; experience using medical record software applications and data repository systems.
Preferred Experience: Experience as a charge nurse or Case Management role or project-focused experience in leading cross-functional teams, and/or program development; experience in community outreach, community development.
Required Licenses/Certifications: Valid license to practice as a Registered Nurse in the state of California, NIH Stroke Scale Certification, ACLS - Advanced Cardiac Life Support Certification issued by AHA, BLS - Basic Life Support Certification issued by AHA.
Preferred Licenses/Certifications: Clinical Nurse Leader certification, certified as a Neuroscience Registered Nurse, Stroke Certified Registered Nurse, or Neurovascular Registered Nurse Certification.
PAY RANGE
$58.74 to $97.91 per hour
The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate’s experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization. Alameda Health System also offers eligible positions a generous comprehensive benefits program.
