Overview

Directs, coordinates, manages, and oversees the activities of ESPRIT clinical staff to ensure the highest level of patient-centered care, including quality and financial outcomes. Collaborates with ESPRIT leadership and other VNS Health departments and stakeholders in the development, design, and implementation of care management program for the Practice. Ensures evidence-based standards of care are integrated into practice. Provides medication management and assessment on an ongoing basis. Provides consultation and education to ESPRIT staff and other agencies and disciplines, including other VNS Health services. Ensures staff adhere to current State and Federal rules and regulations and practice within the scope of practice guidelines for clinical operations including maintaining appropriate licensure. Works under general direction.

What You Will Do 

  • Collaborates with program management to ensure the provision of the highest quality health services to patients. Participates in the formulation and update of clinical and administrative policies and procedures and the preparation of policy and procedure manuals.
  • Collaborates with Medical Directors to maintain current evidence-based clinical policies, procedures, and standards of clinical practice. Supports the integration of evidence-based care practices for a specific program into protocols/policies, consultation strategies, staff development plans, continuous quality improvement initiatives and research for clinical nursing staff.
  • Ensures program services address patient needs. Establishes and communicates protocols and standards of care for a cultural and demographic diverse population; provides intervention guidelines based on these population health needs.
  • Analyzes utilization to ensure visits are made according to guidelines and clinical outcomes best practices. Ensures audits are done per regulatory requirements.
  • Participates on interdisciplinary and interdepartmental work groups that focus on new and existing clinical projects and programs that provide services to patients in the Palliative Care and Annual Wellness Visits.
  • Supports and provides comprehensive advanced nursing care guidance to ESPRIT clinical staff, including physical examination, comprehensive history, screening for physical and/or psychological conditions, emergent interventions, pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling, and patient education.
  • Provides clinical supervision and oversight for field staff, including onsite observation of direct patient care to ensure quality of services. Evaluates performance and reports assessments to clinical management; works with management to set accountability mechanisms and long-term correction standards.
  • Provides and ensures advanced practice nursing care for patients in accordance with current State and Federal rules and regulations for Nurse Practitioner scope of practice and national standards of care, which includes, medical assessments and other assessment and interventions as warranted, as needed.
  • Participates in developing and delivering continuing education programs for clinical staff regarding newly established/revised program specific clinical protocols for effective patient care services.
  • Educates clinicians to follow best practices in utilization management/care management activities Evaluates staff practice patterns on an individual and team level to identify gaps and improve communication skills and clinical practice.
  • Maintains patient records in compliance with VNS Health, state, city, and federal requirements.
  • Participates in VNS Health and community programs and education, as requested.
  • Performs all duties inherent in a managerial role. Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires promotes, and terminates staff and recommends salary actions as appropriate.

Qualifications

Licenses and Certifications:

  • Current ANCC or AANP certification as an adult, family or geriatric nurse practitioner required
  • Licensure as a Nurse Practitioner in the State of New York required
  • DEA and prescriptive privileges required
  • Credentialed status with ESPRIT Medical Care and associated managed care plans required
  • Maintains NPI, Medicaid and Medicare provider numbers
  • Valid driver's license may be required, as determined by operational/regional


Education:

  • Master's Degree of Science in Nursing, or other graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP licensure required


Work Experience:

  • Minimum of four years of experience as a nurse practitioner utilizing full scope of practice required
  • Minimum of one year managing clinical staff preferred
  • Experience working within a healthcare organization and/or developing innovative models of care/services to healthcare systems or providers required
  • Demonstrated understanding of case/care management, population health, disease management and transitional care standards and practices required
  • Clinical home care experience required
  • Ability to work effectively with a wide range of individuals and constituencies in diverse communities required

Pay Range

USD $122,300.00 - USD $164,000.00 /Yr.
