The community transitions program, Care Transitions Clinical Administrative Coordinator role is responsible for conducting initial triage and administrative intake of members, managing admission and discharge notifications and coordinating with clinical teams to support care transitions. The role involves accessing and updating applications to document member case activities as well as supporting intake and notification in alignment with organizational protocols and regulatory standards. Your role allows members to receive timely coordination of services. This is a fast-paced high-volume environment, virtually from their own home.

This position is full time Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7:30am - 4:00pm CST. It may be necessary, given the business need, to work occasional overtime.

We offer 2-3 weeks of paid training. The hours during training will be 7:30am to 4:00pm, Monday - Friday. Training will be conducted virtually from your home.

Primary Responsibilities:

  • Manage member referral creation from admissions, discharges to ensure continuity of care in coordination with clinical teams
  • Daily work activities include member case assignments to determine transitions program eligibility
  • Follow policies and procedures aligned with regulatory and organizational standards
  • Accurately documents pertinent information regarding admissions and discharges from an inpatient setting once discharged to a home or community setting
  • Identifies gaps in workflow from initial admission/discharge case creation and referral documentation
  • Recognize that quality is a core component of all work performed
  • Navigate and operate multiple technology platforms efficiently
  • Maintains confidential health information in compliance with state and federal regulations, including HIPAA.
  • Maintain schedule adherence and demonstrate reliable attendance
  • Consistently meets metrics, for quality, productivity, and outcomes

Required Qualifications:

  • High School Diploma / GED OR equivalent work experience
  • Must be 18 years of age OR older
  • 1+ year of healthcare experience and/or social work/community outreach/advocacy experience
  • 1+ year work with the Medicare and/or Medicaid, DSNP, CSNP Population
  • Work experience using Microsoft Word (edit, create & save documents), Microsoft Excel (sorting & filtering data), and Microsoft Outlook (email, folders, attachments and calendaring)
  • Must have capability to work from home (dedicated workspace, high-speed internet available in the area, and able to hardwire to a modem). Must be willing to work in the local office if/when system or internet issues occur.

Telecommuting Requirements:

  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $20.00 - $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
