Registered Nurse III - Admissions Nurse
Job Description
MVH at Chelsea is actively seeking an Admissions Nurse (Registered Nurse III) to join their team.
In this role, you will provide clinical oversight for the Admissions Discharges Transfers (ADT) Team with coordination of new admissions and intra-facility bed/care coordination. You will complete preadmission screening assessments, serve as a clinical liaison with outside agencies, supervise other nurse leaders surrounding ADT and hospice care related processes and services, collect and analyzes related data, and assist with direct Veteran care when needed.

Duties & Responsibilities:
- Provides consultation to all departments on admission, readmission and hospice related issues
- Maintains records of veterans with hospice services
- Prepares admission and hospices services reports and presents them to the ADT, Nursing Administration and Executive Teams
- Coordinates quarterly meetings with hospice agencies to review quality of care and delivery of services
- Compiles data and keeps accurate records of all hospice care providers, admissions, discharges and transfers and reports results to the Nursing Administration and the Executive Team
- Responsible for participating as the clinical liaison on the EMR team.
- Develops and revises policies with departmental supervisors and helps with implementation
- Leads preadmission process to include preadmission assessment and preparation, identification of needed equipment (i.e. specialty beds or seating surfaces), and potential need for additional services (i.e. rehabilitation, podiatry, etc.)
- Gathers all current medication lists and ensures accuracy. Reconciles medications in preparation for admission
- Facilitates preadmission communication between the Veterans' Home and prospective families
- Supports veteran residents and family through the admission process, introduces family and veteran resident Charge Nurse and assigned Social Worker
- Ensures compliance with regulations, standards, and guidelines of all regulatory agencies, including the Joint Commission, Centers for Medicare and Medicaid (CMS), the Department of Veterans Affairs, and all other licensing agencies regarding the delivery of nursing care services
- Maintains good interdepartmental relations utilizing professional communication
- Acts as liaison between ADT, nursing leadership and other departments
- Assists in formulating, reviewing and revising veteran care policies/procedures
- Institutes changes regarding veteran environment for safety or bed control issues
- Continues professional growth by attending workshops, seminars and work-related courses
- Performs other related duties as required
Qualifications:
- Knowledge of current evidence-based practice.
- Knowledge of and previous experience with statistical methods relevant to the collection, analysis, and reporting of data.
- Knowledge of age-specific considerations in the practice of nursing.
- Leadership skills including effective verbal and nonverbal profession communication, critical thinking, conflict management, and effective decision making.
- Effective change management skills including the application and knowledge of change theories and models to promote high quality care using current best practices and the translation of nursing research.
- Knowledge of principles and practices of health care practices and methods in evaluating the quality of care and nursing practice.
- Ability to maintain confidentiality of veterans' protected health information.
- Expert level physical assessment skills and clinical knowledge including pathophysiology and pharmacology with a focus on the geriatric and Veteran populations.
- Ability to supervise, engage, and lead health care employees utilizing sound performance management practices including coaching and mentoring.
- Utilization of emotional intelligence when interacting with employees, colleagues and administrators and the ability to maintain professional, respectful, and confidential communication.
- Ability to create and maintain harmonious working relationships with others.
Preferred Qualifications:
- Knowledge of and experience with quality assurance and process improvement including LEAN, Six Sigma, TeamSTEPPS, PDSA, root cause analysis and failure mode analysis.
- Highly proficient computer skills including the use of Microsoft Office tools such as Excel and Access to collect, interpret and present data.

Applicants must have at least (A) one year of full-time or equivalent part-time, experience as a registered nurse in a recognized hospital, clinic or medical facility, and (B) six months of which must have been in a supervisory, administrative or managerial capacity.
SPECIAL REQUIREMENTS FOR ALL LEVELS IN SERIES: Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
Recent graduates from an approved school of nursing must have applied for the first available registered nurse examination.
Based on assignment, possession of a current and valid Massachusetts Motor Vehicle Operator's license may be required.
