POSITION DESCRIPTION[1]
POSITION: Post Release Counselor (M/F)
ADDRESS: 365 Main Street, Worcester, MA 01608
I. GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES:
To implement case management and referral services for inmates pre and post release on a consistent basis in accordance with Inmate Services Division standards, policies, and procedures.
II. ESSENTIAL FUNCTIONS:[2]
- To assess basic physical, social, educational, legal and service needs of inmates and coordinate care with other inter-agency and community based providers;
- To effectively communicate and develop positive professional relationships with staff, co-workers, and outside professionals while completing all required documentation;
- To work effectively with community based resources, referral networks, and public/private systems of care
- To carry a caseload and maintain records on each client on caseload
- To ensure that case management needs of inmates are met through provision of appropriate referrals for community based services;
- To provide appropriate case management services that match the needs of inmates;
- To develop and facilitate implementation of after-care plan;
- To attend team and case conferences and collaborate with other providers for designated inmates;
- To provide outreach as designated to community-based inmates;
- To foster an atmosphere of cooperation between inmates and staff;
- To observe conduct and behavior of inmates and to report any unusual occurrences to supervisor;
- To report for work on time and appropriately attired;
- To work harmoniously, cooperatively and courteously with others at all times;
- To perform other duties as assigned;
- To maintain regular attendance.

III. MINIMUM QUALIFICATIONS :
A. Education and Experience :
Bachelor's Degree from an accredited college in a relevant field or with an Associates License (LSWA) license.

B. Special Requirements :
None.
C. Knowledge, Ability and Skill :
Ability to communicate clearly and concisely, in writing and orally
Ability to maintain accurate and detailed records
Ability to talk to others and to effectively convey information
Ability to listen to and understand information and ideas presented through verbal and written communication
Beginner skill in the use of personal computers, particularly word processing
General knowledge of support agencies such as Alcoholics Anonymous, etc.
General knowledge in the effects of drugs and alcohol
D. Physical Qualification s :
Minimal physical effort demanded when performing functions under typical clinical setting.
Frequently required to sit, talk and hear.
Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus.
IV. JOB ENVIRONMENT :
Clinical setting in prison environment.
V. SUPERVISION RECEIVED :
Direct supervision by Director of Regional Reentry Center and Director of Inmate Services

None.
