The Department of Developmental Services (DDS) is searching for an enthusiastic, motivated, reliable individual with a strong work ethic to fill a Human Service Coordinator I (HSC I) position for the Quality Enhancement Department (Q&E) at the Wrentham Developmental Center (WDC). The Service Coordinator monitors the safety and wellbeing of assigned individuals and advocates in regard to their human, civil, and legal rights through the implementation of support services, the Individual's service plan (ISP), and other necessary means.
Duties and Responsibilities: (these duties are a general summary and not all inclusive.)
- Coordinate and facilitate the development and progress of Individual Support Plans (ISP) in accordance with the policies for the Agency and Facility, along with the Federal and State Regulations.
- Monitor the delivery of the ISPs through onsite observations, consultation with staff, and review of resident related documentation.
- Facilitate case conferences, ISP modifications, clinical team meetings to review and resolve problematic areas to enhance resident progress and prevention of regression.
- Assess, evaluate, and document on a periodic basis, the appropriateness and/or need for modification of the Individual Support Plan (ISP).
- Maintain regular contact with Individuals, Families/Guardians, and providers to ensure resident participation.
- Utilize the communication method needed for assigned caseload.
- Provide outreach and coordinate referrals to community services and other state agencies.
- Provide up-to-date record keeping - both electronic and hard copy and maintain documentation for waiver or non-waiver services.
- Organize clinical team meetings to identify resident needs and develop plans to meet those needs.
- Coordinate community placement and facility transfer meetings.

Preferred Qualifications:
- Proficiency with Microsoft programs.
- Must possess current and valid Class D Motor Vehicle Operator's License;
Agency Requirements:
- Must be at least eighteen (18) years of age.
Schedule Information:
Full-time - 40-hour weekly position.
Schedule Monday through Friday - 8:00 am to 4:30 pm
Days off - Saturday and Sunday
Flexibility with work schedule is required.
Special Requirements:
- Candidate must have one of the following Licenses or Degrees to meet the minimum Federal requirements of (Title XIX) for Qualified Intellectual Disability Professionals (Q.I. D.P.) formerly (Q.M.R.P.)
- One of the following is required for Qualified Intellectual Disability Professional assignments in the Department of Developmental Services:
- A current and valid Massachusetts license or registration as a physical therapist, speech-language pathologist, occupational therapist, audiologist, registered nurse, dietitian, or social worker, OR
- A Bachelor's or higher degree in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, or physical education.

Qualifications

Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work.
Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range .
