POSITION: Case Manager
Salary $58,000
I. GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES:
The Case Manager is responsible for providing assistance to individuals referred to the Southern Worcester County Resource Center. Support is provided in accessing mental health, substance abuse, employment, and/or educational services.
II. ESSENTIAL FUNCTIONS: [2]
1. To assess basic physical, social, educational, and service needs of clients and coordinate care with other inter-agency and community based providers;
2. To maintain an active caseload providing guidance and procuring additional services as needed;

3. To compile and share with the team detailed information about assigned clients in order to effectively create the treatment plan;
4. To work with internal departments and/or external agencies to gather documentation needed for treatment plan reviews;
5. To work with the client on a regular basis using professional judgment and discretion to implement the established treatment plan;
6. To consult and cooperate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals;
7. To participate in regular staff meeting, supervision and clinical meetings, as specified by Program Manager.
8. To maintain a high level of ethical conduct regarding confidentiality and professional stature;
9. To maintain records of correspondence and meetings on all cases and matters
10. Maintain working relationships with assigned courts. Ie.) Probation Depts., Judges, District Attorneys.
11. Attend and participate in community events.

Qualifications
III. MINIMUM QUALIFICATIONS:
A. Education and Experience :
1. Bachelor's degree social work, criminal justice, human services; counseling or related field required. Master's degree preferred.
2. Five years of experience in a behavioral health or case management role; or
3. Any equivalent combination of education and experience.
B. Special Requirements:
• Must have valid Massachusetts Driver's License.
• M ust have excellent verbal and written skills.
C . Knowledge, Ability and Skill:
• Ability to communicate clearly and concisely, in writing and orally.
• Ability to maintain accurate and detailed records.
• Ability to talk to others to effectively convey information.
• Ability to listen to and understand information and ideas presented through verbal and written communication.
• Ability to facilitate groups.
• Ability to complete multiple projects in a timely and accurate manner.
• Ability to make presentations.
• Ability to use various software applications.
• Ability to work independently and work with other departments and outside agencies.
• Skill in time management and organization.
• Ability to work varying hours and shifts to meet program needs.
• To support the program staff with scheduling and other auxiliary tasks to support the goals of the center.
D . Physical Qualifications :
• Minimal physical effort required.
• Ability to operate a computer and other typical office equipment.
I. JOB ENVIRONMENT:
Work is generally performed under typical office and correctional facility conditions. Has access to confidential information.
II. SUPERVISION RECEIVED:
Work is conducted under direct supervision of the Program Manager. Work may be performed with some measure of initiative and independence.
