Job Description
Facilitates patient admission, discharge, intra-hospital and inter-facility transfer processes through clinical triage. Acts as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into the organization. Provides clinical expertise, close medical communication and quality assessment for the successful referral and transfer of patients to the facility.
Job Responsibility
- Promotes optimum utilization of hospital beds; conducts rounds and meets with staff regarding bed availability and current census.
- Assigns patients to available beds to avoid delays in patient arrival to units.
- Communicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely manner.

Job Qualification
- Bachelor's Degree in Nursing (BSN) required.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
- 2-5 years of technical experience and 0-2 years of leadership / management experience, required.
