Case Manager, Promise Academy Social Work

Harlem Children's Zone (HCZ) - a world-renowned education and poverty-fighting organization based in New York - seeks an enthusiastic, dedicated, and mission-aligned Social Worker to join the Social Services department at Promise Academy Charter School.

Under the leadership of the Director of Social Services, the Case Manager will support the school community by connecting students and families to external supports and resources, providing support and guidance through the process of being connected, and ensuring that the quality of services meets the needs of the students and families. The Case Manager will develop relationships with external partnerships to link to quality services more effectively and efficiently, particularly regarding mental health, housing insecurity, food insecurity, and other concrete resources.

We are currently seeking a Case Manager to support our Promise Academy Social Work Department. The ideal candidate will bring a passion for the mission of Promise Academy and HCZ, an understanding and respect for the culture of all residents of Harlem, and experience working in a school setting with a love for children.

Minimum Qualifications

  • Masters Degree of Social Work
  • One or more years of relevant experience is required

Who you are

  • A master's degree in Social Work or a closely related field is required
  • Minimum 1-2 years of experience in case management/case planning
  • Knowledge of mental health, trauma-informed practices, child development, social service concerns, and community resources that reflect the social, cultural, and economic diversity of our community
  • Ability to work in a fast-paced environment while demonstrating excellent writing, communication, and interpersonal skills
  • Must be detailed-oriented and possess strong organizational skills
  • Strong computer skills, including PC literacy in a Windows environment, Microsoft Office Suite, and the ability to learn technology quickly
  • Some evening and weekend hours may be required

What you'll do

  • Provide support to the School Social Work team.
  • Coordinate and support with crisis response as needed for critical incidents.
  • Connect students and parents to supportive services as needed to address identified academic and wellness concerns through collaboration with School Social Workers
  • Serve as a liaison/advocate for students and parents with community resources and referrals.
  • Develop/maintain positive working relationships with community referral sources.
  • Research and consult on community resources to meet the trending needs of participants.
  • Maintain daily case note records.
  • Participate in Promise Academy events and initiatives to build rapport with students and families and provide resources as appropriate.
  • In coordination with the School Social Work team, plan, develop, and implement community-based mental health and wellness promotion, education initiatives and materials.
  • Explore and coordinate creative strategies to enhance the community's knowledge of emotional health (i.e. psychoeducation handouts, videos, etc)
  • Participate in weekly supervision meetings and professional development opportunities.
  • Special Projects and other duties as deemed necessary by program leadership.
  • Apply a humanistic approach that promotes respect, empathy, integrity, self-awareness, and compassion.

Schedule

  • Monday-Friday is a must
  • 8 am-4 pm, 9 am-5 pm, or 10 am-6 pm

The annual salary range for this position is $55,000-$60,000, with a comprehensive benefits package including employer-covered health insurance.
