General Purpose of the Job
The Vice President, ESKD Hospitalization Strategy is responsible for developing and executing a comprehensive strategy to reduce hospitalizations among DaVita's End-Stage Kidney Disease (ESKD) patients. This executive leader will work cross-functionally with clinical and operations leaders to drive innovation, enhance clinical programs, and ensure best-in-class patient care and outcomes. This role is pivotal in helping achieve one of DaVita's top clinical priorities: reducing ESKD hospitalizations by 10% by 2030.

Essential Duties and Responsibilities:
Strategic Initiatives
- Define and lead the overarching strategy to reduce hospitalizations across DaVita's ESKD patient population.
- Establish both short- and long-term plans aligned with organizational goals.
- Design, pilot, and scale interventions aimed at improving patient outcomes and operational efficiency.
- Lead a large, multidisciplinary team focused on optimizing critical clinical areas such as fluid management, access, AKI, and adequacy.
- Identify and operationalize required activities for various care team members (RNs, social workers, physicians, etc.), including supporting processes, training, technology, and reporting infrastructure.
- Prioritize and execute annual clinical initiatives that deliver measurable improvements in patient care.
- Continuously assess performance metrics and develop action plans to address gaps.
- Evaluate and engage external vendors to supplement and enhance care delivery capabilities.
- Implement strong management and accountability processes to ensure execution and results.
- Partner with operations to drive sustained performance improvement across markets.
- Provide vision, direction, and performance oversight for a high-impact team.
- Oversee performance development, retention, team structuring, and employee engagement efforts.
- Partner with managers to build and maintain a culture of high performance and inclusion.
- Facilitate input and alignment through cross-functional workgroups involving Operations, Clinical Services, and the Office of the Chief Medical Officer (OCMO).
- Collaborate closely with the VP of Implementation Strategy and other key partners on deployment of technology, training, and reporting.
- Represent clinical capabilities in strategic discussions with senior leadership and client-partners.
- Cultivate a culture of innovation and calculated risk-taking to drive clinical and business advancement.
- Lead teams effectively through ambiguity, identifying and balancing risks and opportunities.
- Minimum of 7 years of experience in healthcare, with dialysis or chronic disease management preferred.
- Demonstrated success in leading large, diverse teams.
- Proven track record of healthcare innovation and outcome improvement.
- Bachelor's degree required; MBA or advanced degree strongly preferred.
- Strong executive presence and communication skills, with the ability to engage C-level leaders and cross-functional teams.
- High proficiency in Microsoft Office (PowerPoint, Excel, etc.).
- Exceptional analytical, conceptual, and strategic thinking capabilities.
- Strong relationship-building and influencing skills, with a demonstrated ability to build trust and credibility across all organizational levels.
- Strong business acumen and a clear understanding of healthcare markets and financial principles.
- Demonstrated ability to manage multiple high-impact projects concurrently.
- Flexible and adaptable, with comfort navigating ambiguity and change.
