Registered Nurse Field Assessor - In Middlesex and Somerset Counties NJ

And we do it all with heart, each and every day.The RN Field Assessor conducts in-person assessments with members to evaluate their medical needs and support their overall wellness. This role helps members access essential services such as adult and pediatric medical daycare, personal care assistance, nursing facility custodial care, the Personal Preference Program, and MLTSS program enrollment.
Assessors are expected to complete 10-15 assessments per week, depending on demand. Each assessment incorporates information from multiple sources to evaluate all health conditions, including co-morbidities and complex diagnoses that affect functionality. Evaluations also consider the member's work capacity and any related limitations.
Using a holistic approach, the RN Field Assessor determines the need for referrals to clinical resources and collaborates with supervisors and team members to overcome barriers to care. The role follows established case management protocols in alignment with regulatory and company policies.
Strong interviewing and engagement skills are essential to accurately assess the member's health status and needs. Once proficient, the RN Field Assessor will also serve as a mentor to new team members.Required Qualifications
- Must possess reliable transportation and be willing and able to travel up to 75-100% of the time within Somerset and Middlesex counties Must be comfortable completing Field based assessments for all socioeconomic status and settings. Mileage is reimbursed per our company expense reimbursement policy
- Must hold an active unrestricted Registered Nurse license in the state of New Jersey
- 2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
- Successful completion of the NJ Choice certification is a contingency of employment. (As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.")
- Must be able to work Monday - Friday, 8:00am - 5:00pm EST
- Assessment and Case management and/or discharge planning experience
- Managed Care experience
- Crisis intervention experience
- NJ Choice certification
- Clinical interviewing
- Experience using personal computer, keyboard navigation, navigating multiple systems and applications; and using MS Office Suite applications (Teams, Outlook, Word, Excel, etc.)
- Minimum Associates Degree in Nursing required
- Bachelors in Science and Nursing preferred

The typical pay range for this role is:$72,627.00 - $155,538.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
