Location: Remote with field travel in Bergen County and surrounding areas

Are you a compassionate and driven RN looking to make a meaningful impact in your community? Join our dedicated team as a Remote Case Manager, where you'll play a vital role in improving the lives of our members through personalized care coordination and advocacy.

What You'll Do:

As a Case Manager, you will:
  • Conduct in-home, face-to-face assessments using comprehensive tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Programs (D-SNP/FIDE).
  • Evaluate member needs and coordinate services such as adult/pediatric medical daycare, personal care assistance, nursing facility placement, and more.
  • Collaborate with members, families, providers, and interdisciplinary teams to develop and implement individualized care plans.
  • Advocate for members during discharge planning and ensure safe transitions with appropriate services in place.
  • Document assessments and care plans accurately and timely in the electronic health record.
  • Mentor new hires once proficient in your role.
What You Bring:

Required:
  • Must reside in Bergen County, NJ
  • Active, unrestricted RN license in New Jersey
  • 3+ years of clinical experience (hospital, home health, or ambulatory care)
  • Willingness and ability to travel 50-75% within Bergen County and nearby areas
  • Own transportation required
  • Successful completion of NJ Choice Certification (80%+ exam score, training modules, and field mentoring)
Preferred:
  • Experience in Home Health, Hospice, LTSS, Case Management, or Discharge Planning
  • Background in Managed Care, Medicaid, and Medicare
  • Proficiency with MS Office Suite and electronic health records
Education:
  • Minimum of an Associate Degree or Diploma in Nursing

The typical pay range for this role is:$72,627.00 - $155,538.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
