Housing Services Specialist Managed Long-Term Services and Supports (New Jersey)

The Housing Services Specialist - Managed Long-Term Services and Supports is a full-time position, solely dedicated to the New Jersey market. They are responsible for ensuring coordination of Housing Services with all other benefits and services provided to all ABHNJ ( Aetna Better Health New Jersey )/Aetna Assure Premier Plus ( HMO D-SNP - Health Maintenance Organization Dual Eligible Special Needs Plan ) members. The Housing Services Specialist reports to the Housing Services Manager and will work in close partnership with the ABHNJ/Aetna Assure Premier Plus ( HMO D-SNP ), and MLTSS ( Managed Long-Term Services and Supports ) leaders, and all matrixed growth partners.Responsibilities include:
- Assist ABHNJ MLTSS members in identifying, securing, and maintaining community-based housing.
- Make timely referrals to in-network pre-tenancy and Tenancy Sustaining Services housing providers.
- Support the development of the housing components of the members' MCO ( Managed Care Organization ) care plan, including setting measurable housing goals.
- Build and foster relationships with Housing Providers, including contractors for Home Modifications and Remediations.
- Maintain active listing of New Jersey housing resources, including (but not limited to) HUD ( Housing and Urban Development ) subsidized housing, all Department of Community Affairs ( DCA ), New Jersey Housing and Mortgage Finance Agency ( NJ HMFA ) housing program voucher programs, public housing authorities, realtors, and online housing locator resources.
- Maintain regular contact with housing providers to ensure service quality and address any issues.
- Monitor provider performance and member satisfaction, escalating concerns to the Housing Services Manager.
- Participate in quarterly reporting and quality improvement initiatives as required by state officials.
- Stay current on housing resources, eligibility criteria, and regulatory updates.
- Support Housing and Social Impact team's special projects as needed.

- 3+ years of experience in assisting vulnerable populations such as MLTSS, elderly, people with disabilities, and homeless to secure accessible & affordable housing.
- 3+ years of experience with NJ public and private housing resources and stakeholders, including a familiarity with HUD subsidized housing, Department of Community Affairs ( DCA ), New Jersey Housing and Mortgage Finance Agency ( NJ HMFA ) housing program voucher programs, public housing authorities, realtors, and online housing locator resources.
- 3+ years of experience in health care.
- Must reside in New Jersey.
- Must possess reliable transportation and be willing and able to travel up to 40% of the time within New Jersey. Mileage is reimbursed per our company expense reimbursement policy
- Strong analytical and communication skills (written and verbal).
- Excellent organization and time management skills.
- Detail-oriented nature.
- Strong relationship building skills.
- Previous experience utilizing Microsoft Excel.
- Bachelor's degree in social work, public health, human services, or a related field.

The typical pay range for this role is:$50,188.00 - $122,400.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
