Mileage is reimbursed per our company expense reimbursement Position Summary The Case Manager for Managed Long-Term Services and Supports (MLTSS) plays a vital role in coordinating and delivering long-term care services to individuals with disabilities and older adults. MLTSS integrates the principles of managed care with Long-Term Services and Supports (LTSS), aiming to enhance care quality, promote independence, and manage Medicaid expenditures effectively.

In this role, the Case Manager works closely with members, their families, and healthcare providers to assess individual needs and develop personalized care plans. Responsibilities include conducting regular face-to-face visits, ensuring access to home and community-based services (HCBS), and facilitating care coordination across various settings. Key Responsibilities:
- Conduct health assessments and develop individualized care plans.
- Coordinate services to support daily living activities such as bathing, dressing, and eating.
- Facilitate access to HCBS and promote member independence.
- Perform scheduled visits:
- Every 6 months for members in nursing homes.
- Every 3 months for members in assisted living facilities.
- Every 3 months for members in private or family homes.
- Collaborate with providers and families to ensure integrated and efficient care delivery.
- Support the goals of MLTSS, including improved health outcomes, enhanced member satisfaction, and cost-effective service delivery.
- Knowledge of managed care principles and LTSS.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a caseload.
- Commitment to improving the lives of vulnerable populations through compassionate and coordinated care.
- Licensed Clinical Social Worker, or Licensed Social Worker in New Jersey.
- 2+ years Behavioral Health experience/expertise required.
- M ust possess reliable transportation and be willing and able to travel up to 75% of the time in Atlantic and Cape May Counties as well as the surrounding areas. Mileage and tolls are reimbursed per our company expense reimbursement
- Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.
- Case management and/or discharge planning experience preferred.
- Managed Care experience preferred
- Crisis intervention skills preferred
- Master's Degree.

The typical pay range for this role is:$72,627.00 - $155,538.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
