As an RN Health Care Manager, you will play a vital role in supporting individuals with a Seriously Mentally Ill (SMI) designation enrolled in West Valley health homes. This person-centered role focuses on building meaningful relationships, coordinating care, and improving health outcomes through integrated medical and behavioral health services.
You will collaborate closely with behavioral health teams, direct care staff, primary care providers, specialists, and Durable Medical Equipment (DME) vendors to address care gaps and support members with complex needs.Key Responsibilities
Member Evaluation & Care Planning
- Conduct comprehensive assessments using care management tools and data insights.
- Identify high-risk factors and service needs; make appropriate referrals.
- Develop and implement individualized care plans; monitor progress and adjust as needed.
- Engage in in-person collaboration with members and clinical teams to ensure holistic support.
- Apply a holistic, strengths-based approach to overcome barriers and achieve care goals.
- Present complex cases for multidisciplinary review to optimize outcomes.
- Identify and escalate quality of care concerns through established protocols.
- Use motivational interviewing and coaching techniques to empower members in their health journey.
- Support members in making informed decisions and actively participating in their care.
- Ensure compliance with regulatory, accreditation, and organizational standards.
- Maintain accurate, timely documentation of all care management activities.
- Candidates must have a dedicated workspace free of interruptions.
- Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted.
- Residency: Must live in Maricopa County, AZ.
- Licensure: Active and unrestricted Arizona RN license.
- Experience:
- Minimum 3 years of clinical practice.
- At least1 year of case/care management in an integrated care setting.
- Technical Skills: Proficiency in MS Office Suite, including Outlook, Excel, and Word.
- Mobility: Willingness and ability to travel within the assigned region (10-25%).
- Behavioral Health Experience: 1+ year working with behavioral health populations.
- Associate's degree in Nursing (required)
- Bachelor's degree in Nursing (preferred)

The typical pay range for this role is:$60,522.00 - $129,615.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
