RN Referral Coordinator, Hematology/Oncology

Summary:
Referral Coordinator is a registered nurse (RN) responsible for promoting the services and enhancing the reputation of Cook Children's Hematology and Oncology Department with a concentrated focus on relationship building and follow up with referral providers regarding current status of H/O patients. Referral Coordinator will use a collaborative process to plan, assess, implement, monitor and evaluate the options and services required to meet the healthcare needs of the patients using utilization review and case management skills. Referral Coordinator shall deliver competent and skilled care to patients and families according to their identified needs. Nursing care is based on the nursing process and augments the medical plan or treatment. The Referral Coordinator will deliver care with a team-orientation, and emphasis on good customer relations, sound clinical judgement and appropriate decision-making abilities that take into consideration evidenced based practice.
Monday - Friday 8am - 5pm
Duties:
- Review incoming referral requests- WQ, Fax, Inbox message, E-Mail (log all referrals)
- Use critical thinking for urgency of referral
- Call referring for missing documentation
- Once all information is received send to be reviewed by physician
- Collect reviewed green sheets - update log with scheduling instructions
- Call referring office with any requests from reviewing physician (new labs, imaging discs, does not need to be seen, penicillin letters, newborn screens etc.)
- Take green sheets to be scheduled
- Q2 weeks follow up on green sheets with missing information
- Call referring physician after patient has been seen and give update. (Fax notes and labs if needed)
- Follow up on outside requested records from outside facilities
Education & Experience
- Bachelor's degree required.
Licensure, Registration, and/or Certification
- Current licensure to practice professional nursing in Texas.
- Current BLS.
- Preferred Certification in Oncology Nursing, OCN, ACON, or CPHON.
