Additional Information:
Provides support for all Service Coordination Department functions which include utilization management, service coordination, and population health management. This position also provides health education and care coordination services for CCHP members telephonically, in home, or health facility settings.
Education & Experience
- High School graduate minimum. Associate Degree preferred but not required.
- Three years' experience in a managed care or medical office/hospital environment.
- Bi-lingual (English/Spanish) Required. Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.

Knowledge, Skills & Abilities
- Superior verbal and written communication.
- Strong customer service skills, strong phone skills.
- Familiarity with basic medical terminology and ICD-9 and CPT coding.
- Personal computer skills.
- Expertise with various software applications/databases (i.e., Microsoft Office).
- Highly organized, ability to prioritize, and demonstrate moderately complex problem solving skills.
- Ability to work autonomously with minimal supervision in various settings.
- Basic service coordination concepts.
Certification/Licensure:
- Certified Community Health Worker.
