Summary:
Provides support for all Service Coordination Department functions which include utilization management, service coordination, and population health management. This position also provides health education and care coordination services for CCHP members telephonically, in home, or health facility settings.

Duties:
• ur Certified Community Health Worker will provide telephonic support for members of the Cook Children's Health Plan via incoming phone calls, fax, or the provider portal. Provides service coordination (e.g., assist with scheduling appointments, linking to community resources, etc.) to members who have moderate health complexity and communicates with health care providers to promote efficient care delivery. Will work to ensure members receive the physical and behavioral health, pharmacy, and social support services needed. Will provide Spanish interpretation and translation, including medical terminology. Performs outreach activities, including health risk assessments and special projects.

Hybrid position: Can work four 10-hour days 7 am-5:30 pm or Mon - Friday 8 hour days. Will primarily work from home with 2 days per month in office required.
Required Education, Experience & Certification:
- High School graduate.
- Three (3) years' experience in a managed care or medical office/hospital environment.
- Bi-lingual (English/Spanish) Required. Expert proficiency in conversational, reading, writing, and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.
- Certified Community Health Worker certification required.
Knowledge, Skills & Abilities:
- Superior verbal and written communication.
- Strong customer service skills, strong phone skills.
- Familiarity with basic English & Spanish medical terminology and ICD-9 and CPT coding.
- Personal computer skills.
- Expertise with various software applications/databases (i.e., Microsoft Office).
- Highly organized, ability to prioritize, and demonstrate moderately complex problem-solving skills.
- Ability to work autonomously with minimal supervision.
- Basic service coordination concepts.
