Responsibilities:
- Assessment : Evaluate patients for functional limitations and collaborate with patients, families, nurses, physicians, and other health team members to develop a personalized therapy plan.
- Direct Care : Provide therapy services to treat medical and surgical conditions using accepted physical therapy modalities.
- Education : Instruct and supervise patients, families, home health aides, and others on home exercise programs and aspects of patient care.
- Ongoing Evaluation : Continuously assess the patient's condition and adjust the therapy plan as needed.
- Communication : Report significant clinical information to physicians, clinical managers, and other team members. Document clinical evaluations, treatments, and patient responses to therapy.
Qualifications:
- NY Professional License and current registration as a Physical Therapist.
- Minimum 1 year of full-time experience providing physical therapy services.
- Ability to assess and respond to patient and family needs in a home care setting.
- Home care experience is preferred but not required.
- Ability to use technology (such as tablets) for patient documentation and care.
- Experience with Oasis assessments is a plus.
- A valid driver's license and access to a reliable automobile for travel in the field.
Salary:
- $95,000 to $108,000 annually , based on experience.
