You will work in our clinic in Monterey County and provide high-quality, ethical patient care that follows evidence-based medicine within the primary care scope of practice. Patient cases include moderate to complex care, under minimal supervision of other physicians. We are looking for those trained and skilled in comprehensive first contact and continuing care for persons with diagnosed and undiagnosed signs, symptoms, or health concerns not limited by problem origin (biological, behavioral, or social), organ system, or diagnosis.

About Your Work
As the Primary Care Physician in our clinic, you will work closely with the clinical team and be responsible for managing direct patient care using evidence-based medicine and adherence to national and internal practice guidelines and policies. You will advocate on behalf of patients under your care to receive necessary, cost-effective care by coordinating appropriate health care services. You will promote effective communication with patients and encourage the role of the patient as a partner in health care. You will also collaborate with other Altais medical group physicians to drive clinical initiatives across all Altais medical group clinics.

You Will Focus On:

  • Health promotion, Disease prevention, Health maintenance , Counseling, Patient education, Annual Health Assessments (AHA), Chronic Care Management (CCM),
  • Provide direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
  • Collaborate with providers, ancillary personnel, community resources and other interdisciplinary team members to proactively resolve patient needs, including collaborating with other health professionals, and utilizing consultation or referral as appropriate.
  • Leverage the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows and clinical compliance.
  • Perform clinical coding, documentation and closing of encounters for timely submission of services rendered
  • Provide patient advocacy in the health care system to accomplish necessary, cost-effective care by coordination of health care services.
  • Promote effective communication with patients and encourages the role of the patient as a partner in health care.
  • Standards continue to be refined reflecting the scope of services within which PCPs are expected to practice. Board certification must be current. Complete documentation via an electronic medical record is recommended.

The Skills, Experience & Education You Bring

  • Medical degree is required
  • Successful completion of residency in the specialty of Family or Internal Medicine is required
  • Minimum of 4 years clinical experience required Demonstrated knowledge and experience using electronic medical records, Epic experience strongly preferred
  • Active California medical license, NPI and DEA, all in good standing
  • Experience providing evidence based and value-based medicine, serving as patient health advocate.
  • Working in interdisciplinary teams.
  • Applying quality improvement plans
  • Ability to function independently as well as part of a team in a dynamic environment
  • Excellent communication skills and ability to adjust communication styles when working with patients, their families and colleagues.
  • Highly organized, strong time management skills and flexibility to manage multiple timelines and activities.

Final determination of a successful candidate’s starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
