About Your Work
Our NP or PA will perform comprehensive health assessment which include but not limited to the following:

You Will Focus On:

  • Obtain history and perform physical examination
  • Complete review of medicines and related instructions
  • Review home safety and psychosocial needs
  • Fulfill documentation needs for STAR and HCC measures
  • Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
  • Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
  • Maintain patient safety and confidentiality
  • Universal precautions utilized in all patient care
  • Prescribe/order procedures, treatments and medications within scope of practice
  • Develop high level of trust and intimacy with patients and families

The Skills, Experience & Education You Bring:

  • CA Licensed Nurse Practitioner or Physician Assistant
  • 2+ Experience with Primary Care
  • Bilingual in Spanish

Certifications

  • Active and in good standing RN licensure and Nurse Practitioner Certification in the state of California
  • Active and good standing NPI number
  • Active and good standing DEA number from US Drug Enforcement as required per specialty
  • Basic Life Support
  • Preferred: Advanced Cardiac Life Support as required per department/specialty

Final determination of a successful candidate’s starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
