Multi-Systems Specialist (Closes 6/3/2024)

Summary

The Multi-Systems Specialist, with direction from the Multi-systems Supervisor, within the Department of Human Services, supports planning and staffing efforts to serve children and adults who are being served by more than one program area within DHS. The primary operational responsibility is to coordinate and implement coordinated planning for multi-systems involved youth and adults, co-facilitate meetings across a variety of venues and provide technical assistance in procuring and linking appropriate resources. Incumbent also serves as a designee on various projects throughout the program offices.

Essential Duties and Responsibilities

  • Reviews multi-systems referral information and triages referrals assuring appropriate resources and subject matter.
  • Convenes, facilitates, and staffs a multi-systems planning Meetings, depending on needs.
  • Provides Technical assistance and troubleshoots individual cases of youth and adults.
  • Acts as a liaison/broker/technical assistance with DHS program offices/service. providers/caseworkers/managers/contract monitors to assist in the identification of needed services when necessary.
  • Connects and follows up with needed resources identified in multi-systems meeting or TA meetings.
  • Assures that all agreements and action steps determined through the multi-systems team are completed.
  • Ensures that appropriate follow-up and communication to team members of achieved assignments or goals occurs in a timely manner.
  • Inputs and shares notes, action steps and data in a central database repository (Synergy, KIDS, MPER) of all individuals who are being served through the multi-systems team on a weekly or monthly basis.
  • Builds and maintains professional relationships within the Multi-systems arena through the Department of Human Services, Providers and Stakeholders.
  • Assists with tracking and planning for youth in Probation placements, Hospital inpatient stays and RTF placements.
  • Refers to Multi-Systems Supervisor to facilitate Rapid Response referral, Options Meetings and Specialized Placement referrals.
  • Identifies when the Multi-systems Planning calls can not result in viable options/resolution due to lack of resources, readily available resources, regulation issues, etc. These cases are then referred to the Multi-Systems Supervisor to raise awareness to administration within the DHS program offices.
  • Upon admission to a specialized group home coordinates and facilitates 10-day admission meetings and assure case transfer meeting occurs upon admission to the staff lead to which the case will be transferred to.
  • Assure case transfer process is followed when multi-systems team is transferring a case within the team or to another program office within DHS.
  • Works closely with CYF Placement Stability, ODS Promising Practice and OBH Childrens teams on a regular basis.
  • Attends CYF and other program office staffing, as needed.
  • Regular Individual and group supervisions to discuss case concerns and progress
  • Co- located at CYF Regional office at least 2 days per week and will travel to other program office locations as needed.

Education/Experience Requirements

Bachelor's degree in social work or related field, plus a minimum of one year of clinical and case management experience required. Past experience in the development of a multi-systems service design preferred, plus policy, administration and funding experience a plus.
