The MTM Clinical Admin Nurse supports medication therapy management by conducting medication reviews, identifying care gaps, educating patients on safe and effective medication use, and documenting all clinical interactions accurately. The nurse coordinates care with pharmacists and providers, assists with adherence and quality initiatives, ensures compliance with MTM and CMS standards, manages patient outreach and scheduling, and handles administrative tasks within MTM platforms to support smooth clinical operations.

Primary Responsibility:
- Comply with the terms and conditions of the employment contract, company policies and procedures, and any and all directives (such as, but not limited to, transfer and/or re-assignment to different work locations, change in teams and/or work shifts, policies in regards to flexibility of work benefits and/or work environment, alternative work arrangements, and other decisions that may arise due to the changing business environment). The Company may adopt, vary or rescind these policies and directives in its absolute discretion and without any limitation (implied or otherwise) on its ability to do so
Required Qualifications:
- Bachelor's degree of Nursing + Active USRN (mainland only) & PHRN license
- 2+ years of hospital experience preferably ICU/MS/ER/Gen - Med Recon, Adult care
- MTM target patient w/ multiple diseases with multiple medications which comes with high annual spend
- Candidate should know drugs, but they need to have knowledge of prior indications of medications
- Proven ability to think on their feet because they are talking to patients
Preferred Qualification:
- MTM and diabetes certification
