The Healthy First Steps and NICU (HSS) Clinical Coordinator RN role provides comprehensive care management for a high risk specialty population, addressing medical, behavioral health, and social determinants of health needs. This role is dedicated to improving maternal and infant outcomes through proactive engagement, evidence based education, early identification of risks, and connection to appropriate clinical and community resources.

For all hires in the Minneapolis or Washington, D.C. area, you will be required to work in the office a minimum of four days per week.

Primary Responsibilities:

  • Identify medical, behavioral, and social risk factors and develop individualized plans of care to address identified needs
  • Conduct outbound and inbound calls with members in the specialty population to assess current health status and determine care coordination needs
  • Initiate referrals to community resources to address social determinants of health, ensure referral closure, and track outcomes
  • Collaborate with Medical Directors on complex or challenging cases requiring clinical escalation
  • Provide evidence-based education to support member self-management of identified health risks
  • Coordinate care and connect members to appropriate services, both internally and externally
  • Partner with members to establish and maintain a plan of care focused on achieving optimal health outcomes
  • Manage assigned queues to ensure timely follow-up and proactive outreach to the specialty population
  • Coordinate care with healthcare providers and other care team members as needed
  • Document assessments, findings, interventions, and outcomes in the internal care management system according to policy requirements
  • Comply with departmental policies as well as internal, external, and regulatory requirements (including legal and compliance standards)
  • Educate and support non-clinical team members involved in member care coordination
  • Perform other related duties as assigned to support program and organizational goals

Required Qualifications:

  • Current, unrestricted RN license in the state of Tennessee or compact state licensure
  • 3+ years of work experience with assigned specialty population
  • 3+ years of clinical experience in a hospital, acute care, home health/hospice, or case management
  • Intermediate computer/typing proficiency to enter and retrieve data in electronic clinical records as well as experience with email, internet research, use of online calendars, and other software applications
  • Unrestricted driver's license and reliable transportation within the service area
  • Proven problem-solving skills, including the ability to systematically analyze problems, draw relevant conclusions, and devise appropriate courses of action
  • Proven ability to communicate complex or technical information in a manner that others can understand, as well as the ability to understand and interpret complex information from others
  • Ability to travel in assigned region to visit members in their homes and/or other settings such as hospitals or providers' offices
  • Must reside in Tennessee (can be region-specific)

Preferred Qualification:

  • BSN degree
  • Case Management experience including Certification in Case Management
  • Experience in telephone triage or call nurse services
  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
  • Home care/field-based case management and/or experience in Home & Community based or Long-Term Care services delivery
  • Medicaid, Medicare, Managed Care experience
  • Bilingual skills

  • Proficiency in multitasking, including simultaneous phone and computer-based work while supporting members.
  • Exceptional interpersonal and communication skills, with the ability to engage effectively with both internal and external stakeholders.

  • Flexibility in work schedule to support program and member needs.

  • Effective resource management skills, ensuring tasks are completed on time and within established constraints using available tools and supports.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.94 to $51.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
