About the Role:
Join McKesson’s Patient Support Programs to help patients start and stay on specialty therapies. As a Bilingual Nurse Case Manager, you’ll serve as the single point of contact for prescribers, clinic staff, and patients—coordinating onboarding, education, and ongoing adherence support across the treatment journey. This field/remote role includes travel to clinics, homes, or virtual visits to deliver injection training, disease/medication education, and to liaise with care teams for smooth, timely access to therapy.
What You’ll Do:
- Act as the primary contact for prescribers and clinic teams to enroll patients and coordinate services.
- Provide patient education (disease state, medication, side‑effect management) and deliver in‑home, in‑clinic, or virtual injection training based on patient preference.
- Support adherence through scheduled follow‑ups; share timely clinical updates with physicians/clinic staff.
- Complete concise, accurate documentation; conduct chart reviews as needed for reimbursement navigation.
- Collaborate with McKesson pharmacy colleagues, manufacturer partners, and clinic teams to remove barriers to care and escalate issues beyond program scope when required.
Basic Requirements:
- Bilingual: Fluent in French and English (spoken and written).
- Nursing license in Ontario or Quebec (or both). RN strongly preferred; RPN/LPN and NP considered.
- 2+ years of relevant experience (or an equivalent combination).
Preferred Skills/Experience:
- Direct experience in case management, patient education, or specialty therapy support.
- Experience in immunology, rheumatology, dermatology, or gastroenterology; familiarity with biologics and pre‑treatment work‑ups (e.g., TB testing, imaging, labs).
- Comfortable delivering injection training and using adult‑learning principles to tailor teaching.
- Strong relationship‑building, documentation, and MS Office skills; ability to prioritize across multiple stakeholders in a field‑based environment.
Physical Requirements:
- Territory location: Quebec and/or Greater Toronto Area (GTA).
- Program hours are 8am to 8pm EST.
- Job hours are: 9-5PM EST, Monday to Friday (subject to change)
- Occasional travel may be required.
