The RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member's overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment.

  • Minimum of 20 assessments per week dependent on need, may be more. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member's level of work capacity and related restrictions/limitations.
  • Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality.
  • Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives. Utilizes case management processes in compliance with regulatory and company policies and procedures.
  • Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation.
  • The RN Assessor will also be expected to mentor new hires on the team, once, a level of proficiency has been attained in their role.

  • Candidate will work from home (WAH) out of their residence in NJ
  • Have reliable phone and hard-wired internet service.
  • Work schedules are determined by the Leader based on business needs and are not affected by the work model or location . Mon-Fri 8-5 EST
  • Colleagues must be reachable via phone, videoconference, and email during work hours (8 Hours per Day) as directed by their Leader.
  • Personal responsibilities, including dependent care, must not interfere with work duties. Dependent care during work hours is generally not allowed.

  • Meet any additional reasonable requirements set by the Leader or Company.
Field Requirements:
  • A field-based Colleague regularly works in multiple company locations. Those locations may include company locations, client locations, facilities (nursing home, rehabilitation centers, etc.), member homes, and/or community locations.
  • Unlike office-based and hybrid Colleagues who generally work in the same company location during their work week, field-based Colleagues
Required Qualifications:
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time within Ocean and Burlington Counties.
  • Must be comfortable completing Field based assessments for all socioeconomic status and settings.
  • Mileage is reimbursed per our company expense reimbursement policy
  • Must hold an active unrestricted Registered Nurse license in the state of New Jersey
  • 2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
  • Successful completion of the NJ Choice certification is a contingency of employment. (As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.")
  • Must be able to work Monday - Friday, 8:00am - 5:00pm EST
Technical Skills Required:
  • Proficient in Multisystem Navigation: Comfortable using multiple monitors and systems simultaneously to streamline tasks and improve efficiency.
  • Effective Multitasker: Skilled at talking and typing concurrently while managing various responsibilities with focus and accuracy.
  • Basic Technical Troubleshooting: Able to set up and connect essential hardware (monitors, keyboards, etc.) and resolve common technical issues independently.
  • Digital Collaboration & Documentation: Experienced in electronic documentation and virtual collaboration tools to support remote teamwork and communication.
  • Highly Organized & Results-Oriented: Maintains a structured approach to work, ensuring tasks are completed on time and in alignment with performance metrics.
  • Metric-Driven Performance: Demonstrates accountability by consistently meeting or exceeding established goals and expectations.
Preferred Qualifications:
  • Assessment and Case management and/or discharge planning experience
  • Managed Care experience
  • Crisis intervention experience
  • Clinical interviewing
Education Requirements:
  • Minimum Associates Degree in Nursing required
  • Bachelor's in Science and Nursing preferred

The typical pay range for this role is:$72,627.00 - $155,538.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.Our people fuel our future.
