MGR - CARE COORDINATION

Under the direction of the Director, Ambulatory Care Coordination, this position has responsibility and accountability for managing, organizing, and implementing actions to ensure that Care Coordination services are provided in a professional, ethical, and responsible fashion across YNHHS and in other stakeholder settings. The Manager of Care Coordination will provide guidance, consultation, and support to the Care Coordination team. The key focus areas include: disease management, transitions of care, complex care management, risk stratification, promotion of preventive medicine and closing gaps in care, in addition to wellness programming for broad audiences (e.g. educational events/seminars, screenings, lifestyle coaching). The role includes program development, program evaluation, tracking data across multiple data points, facilitating improvements in the delivery of ambulatory care coordination services, and reporting on the quality, effectiveness, and cost effectiveness of all programs. The Manager of Care Coordination must be a dynamic individual with a passion for improving the quality of healthcare delivery across the continuum of care. The Manager, Care Coordination, will support the vision of Yale New Haven Health to deliver the highest quality accessible health care to all patients.

Responsibilities

  • 1. Oversight and Management
    • 1.1 Manages teams of care coordinators, wellness coaches/coordinators, patient navigators and other staff as programs develop.
  • 2. Performance Improvement
    • 2.1 Supports program staff to reduce variation and standardize programs, interventions, and operational effectiveness across all ambulatory care management settings according to best practices.
  • 3. Strategic Planning
    • 3.1 Works closely and in collaboration with leadership in many YNHHS functional areas to set the strategic direction for program development, execution, and resource deployment.

Qualifications

EDUCATION

 

Master?s Degree in business, public health, nursing, integrative health, healthcare, administration, or other related Master?s Degree strongly preferred. Valid CT SW license, RN license or Physician Assistant license required. Bachelor?s degree required. Advanced training in performance improvement and motivational interviewing is preferred.

 

EXPERIENCE

 

5 years of nursing experience required with increasing responsibilities and experience in managing care across the continuum.

 

LICENSURE

 

Valid CTR SW, RN, APRN< RD or PA Licensure required.

 

SPECIAL SKILLS

 

A passion for improving the delivery of healthcare in the ambulatory setting and across the continuum of care. Superior communication skills, including presentations. An ability to work collaboratively with and influence individuals at varying levels within the organization. Past experience delivering results in a matrixed work environment. Must be flexible, team oriented, able to work independently and autonomously, a critical thinker, and excited for a steep learning curve.
