CLINICAL PHARMACY SPECIALIST - MULTISPECIALTY

Clinical specialist supports the vision, mission, and goals of YNHH and the Department of Pharmacy and demonstrates proficiency in clinical pharmacist skills. Clinical Specialist must have met all general requirements of Clinical Pharmacist and Clinical Pharmacist 2 positions in addition to specific pharmacy practice, drug information, educational/scholarly, and leadership requirements.

Responsibilities

  • 1. Drug Therapy Management
    • 1.1 In a timely fashion, reviews and verifies all medication orders to ensure safe, accurate, cost effective drug use.
  • 2. Interpretation of Clinical & Pharmacokinetic Lab Data
    • 2.1 Demonstrates knowledge of pharmacokinetic and pharmacodynamic principles and applies to recommend changes in medication regimens.
  • 3. Medication-related issue identification and resolution
    • 3.1 Consistently documents Medication Use Variances
  • 4. Drug Information
    • 4.1 Provides drug information to physicians, nurses, and other health care professionals.
  • 5. Communication and Service Requirements
    • 5.1 Understands role as integral member of the health care team.
  • 6. Education/Scholarly Activities
    • 6.1 Performs patient counseling
  • 7. Leadership
    • 7.1 Consistently attends department meetings
  • 8. Regulatory and Safety Quality Compliance
    • 8.1 Completes nursing unit inspections, override audits, clean and safe rounds, Pyxis sweeps and other regulatory responsibilities as assigned by manager.
  • 9. Drug Distribution Activities
    • 9.1 Oversees the drug distribution process, assuring accurate and timely dispensing of drugs to patients via pharmacy systems (i.e. Pyxis dispensing machines, code carts, etc.).

Qualifications

EDUCATION


Graduation from an accredited College of Pharmacy with a B.S., M.S., or Pharm.D. degree.


EXPERIENCE


Completion of an ASHP accredited PGY1 pharmacy residency and PGY2 pharmacy residency OR equivalent experience (4 post-graduate years in area of practice specialty) is required. Each residency is equivalent to 2 years of post-graduate experience.


LICENSURE


Active Connecticut Pharmacist License. Board of Pharmacy Specialities Certification is preferred.


SPECIAL SKILLS


This individual should demonstrate in-depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Experience in drug utilization review and data analysis is highly desirable. Excellent written and verbal presentation skills are required.
