The Clinical Integration Coordinator will support the Vision of Yale New Haven Health System to enhance the lives of those we serve by providing access to integrated, high-value, patient-centered care in collaboration with others who share our values. Quality clinical care defined by the Institute of Medicine is care that is safe, effective, patient-centered, timely, efficient and equitable. The Clinical Integration Coordinator must be a dynamic individual with a passion for improving the quality of health care delivery in the ambulatory setting and across the continuum of care an ability to build robust, collaborative relationships with multiple stakeholders exemplary project management and organizational skills the ability to thrive in a complex, highly matrixed organization strong communication skills to keep system leadership and stakeholders apprised of program deliverables work autonomously and with minimal oversight, while being a strong team player Working with physicians, management, nurses and staff, the Clinical Integration Coordinator: supports clinical workgroups to reduce variation and standardize clinical care and operational effectiveness according to best practices; supports clinical workgroups in efforts to build clinical effectiveness, become more clinically integrated and improve the patient experience; and assists the clinical workgroups to collect, document and report relevant patient safety and clinical quality metrics.



  • 1. Leads, facilitates and executes projects to improve clinical quality performance and operational effectiveness
  • 2. Designs, implements and manages performance improvement and quality metrics.
    • 2.1 Works with provider and clinical operations staff to develop appropriate documentation regarding safety practices, clinical effectiveness policies and performance improvement.
  • 3. Is responsible for ensuring compliance with and participation in any other necessary public reporting programs.
    • 3.1 Maintains an expertise in additional quality reporting registries and public reporting of quality data as it relates to the ambulatory setting, including but not limited to the PQRS, Meaningful Use and CG-CAHPS programs.
  • 4. Influences and communicates with all levels of clinical, financial and operational management concerning team goals and progress
  • 5. Supports operations by using appropriate information systems to develop integrated clinical and operational data analysis and where relevant, financial analysis
  • 6. Uses established performance improvement methodologies and frameworks to drive projects, including but not limited to PDSA, LEAN, Six Sigma, Influencer, PMBOKand CAP/Workout
  • 7. Participates in YNHHS-wide task forces and committees to provide performance improvement input; makes focused recommendations with an eye for YNHHS-wide clinical care transformation and outcomes improvement
  • 8. Prepares written abstracts, posters and presentations for projects for presentation at the annual YNHHS JAZ Patient Safety and Clinical Quality conference and within/outside of the organization as appropriate.
  • 9. May assist with applied research projects that are related to clinical integration, patient safety and quality improvement.




Master's degree in business, public health, nursing or other related field strongly preferred. Valid CT RN license strongly preferred. Bachelor's degree required. Advanced training in performance improvement methodologies including LEAN is preferred.




At least five (5) years of experience working within a health care setting with progressively increasing responsibilities. Experience in data analysis, statistics and performance improvement is required.




CT RN license strongly preferred.




A passion for improving the delivery of health care in the ambulatory setting and across the continuum of care. Superior communication skills, including presentations. An ability to work collaboratively with and influence individuals at varying levels within the organization. Past experience delivering results in a matrixed work environment. Must be flexible, team-oriented, able to work independently and autonomously, a critical thinker and excited for a steep learning curve.
