ADVANCED PRACTITIONER: NURSE PRACTITIONER OR PHYSICIAN ASSISTANT
PRIMARY CARE OUTPATIENT - INTERNAL MEDICINE
PART TIME OR FULL TIME
HAMDEN, CT (MOVING TO BEAUTIFUL NEW OFFICE IN NORTH HAVEN SPRING OF 2017)
Northeast Medical Group (NEMG) is a medical foundation comprised of over 600 top primary and specialty care physicians throughout Fairfield and New Haven counties and in New York’s Westchester County. As part of the Yale New Health System, one of the country’s leading healthcare systems, NEMG provides physicians with access to advanced clinical and collaborative care of three leadings hospital- Bridgeport, Greenwich and Yale New Haven.
We are seeking an experienced advanced practitioner to work with a seasoned internal medicine physician located in Hamden, CT. This position is part time with potential for full time once office moves to new building in May of 2017.
Part Time Hours: Monday 11:00 a.m. - 7:00 p.m.
Wednesday 8:00 a.m. - 5:00 p.m. (Flexible)
Friday 8:00 a.m. - 5:00 p.m. (Flexible)
Functions independently and may maintain a panel of patients as the assigned primary care provider
Demonstrates excellence in the completion of patient history and physical exams and assessment
appropriate to patient population
Demonstrates proficiency in treatment plans and interventions, working collaboratively with other APCs ,
physicians, or specialists when indicated.
Prescribes appropriate pharmacologic and non -pharmacologic treatment modalities
Serves as a resource for families
Integrates ethical principles in all areas of practice.
Maintains regular communication with patients, families, and colleagues as appropriate regarding progress
and treatment plans.
Educational preparation at the Masters Level. Graduation from an accredited Advanced Nurse
Practitioner program or Physician Assistant Program. Certified with CT State Licenses.
2-3 years clinical experience related to outpatient internal medicine preferred.
Excellent clinical, interpersonal, and organizational skills. Ability to work independently
