The RN functions as part of the primary care team, and reports to the RN Nurse Manager and / or Clinical Services Manager providing care to residents of Long Term Care, Skilled Nursing and other facilities. The RN supports the team by performing activities as outlined below, and as assigned by the RN Nurse Manager or Clinical Services Manager of the team. The RN performs activities that fall within the RN scope of practice. This role works in close collaboration with all of the team members, and may support multiple providers. The RN works under the direction of the provider, and activities are delegated to the RN by the provider, RN Nurse Manager or the team CSM. The RN understands and supports the ISNP model of care, and works in collaboration with the team to meet the goals of the program.

Responsibilities may include duties in the following areas:

INSERVICES

  • Monthly outreach to facility contact to determine inservice needs
  • Keep CSM / NP aware of scheduled inservices
  • Maintain facility - specific inservice tracker
  • Conduct OPTUM 101's at new facilities during off hours for 2 nd & 3 rd shift staff
  • Periodically provide facilities with list of inservices available from OPTUM
  • Participate as appropriate in monthly facility new employee orientation providing an abbreviated OPTUM 101 or "walking rounds" in services

ROUNDS
  • Review daily On Call log & review PNP signout
  • Conduct rounds at facilities during PTO time & as requested by CSM
  • Assess & report to PNP with any issues
  • Round on "red" residents as requested during normal business hours as well as off hours and weekends

CARE ONE & CHARTS / Pathway EMR
  • Data mining in facilities
  • Backfill collection / entry of new effectives
  • Maintain monthly back fill tracker specific to RN building assignment
  • Update data as requested by team NPs
  • Ensure resident charts have proper OPTUM identification as needed

CONFERENCE CALLS / MEETINGS / WEBEX
  • Weekly Organizational Recap call
  • Weekly RN call
  • Monthly Documentation & Coding Webex
  • Monthly designated Committee call
  • Attend CSM Team Meetings
  • All Staff Meeting

MISCELLANEOUS TASKS AS REQUESTED
  • Daily Log Review sent out from 6am log - send to PNP's, CSM's & CA's of specific team
  • Updating / creating Resident Rosters
  • MMSE's (mostly in the 1 st month of opening new facilities)
  • Prior Authorization completions
  • On call fax box clean up - checking 2x / week & sending data to appropriate staff
  • Replacing member face sheets when NP assignments change

Accountabilities
  • The RN reports to the team CSM
  • The RN is accountable for collaborating with, and working under the direction of the NPs and PAs, and the team CSM
  • The RN may be assigned to work with multiple providers, and duties and responsibilities may vary according to market needs
  • The RN functions as part of the overall team, and is accountable for supporting the team in meeting team and market goals and initiatives

Credentialing

  • The RN is responsible for having and maintaining current RN licensure in their state of practice
  • The RN is responsible for having and maintaining current individual nursing home credentialing
  • The RN is responsible for maintaining other certifications such as CPR, etc., as required by the state or market


Required Qualifications:
  • Unrestricted RN license in state of practice
  • 5+ years of experience working as an RN preferred
  • Working knowledge of Medicare / Medicaid regulations
  • Proficient in computer applications including Word, Excel and Power Point
  • Ability to travel

Preferred Qualifications:
  • Bachelor's degree
  • 5+ years of experience in a community based, Home Care, Hospice and / or long term care setting
  • Experience with using an Electronic Medical Record
  • Familiarity with the long term care business environment
  • Experience working with the geriatric population
  • Strong communication skills, written and verbal
  • Strong organization skills and time management skills
  • Work independently in an unsupervised setting
