This is a position for an individual with a bachelor degree in a human services related field and one experience working with adults with mental illness

This position is responsible or the day to day activities of the Med Drop Program located in Redford, Lincoln Park and some hours at Northeast Guidance Center in Detroit, MI.

This is a Full Time Hourly Position with benefits. Hours are Monday 9:30 AM to 6:00 PM. Tuesday through Friday 8:30 AM to 5:00 PM. The salary range is $17.50 per hour to $18.50 per hour.

The Med Drop Services (MDS) Coordinator position is responsible for the day to day activities of the Med Drop program. This position will report to the MDS Program Manager. This position is located in a Site Pharmacy and the MDS Coordinator is expected to follow the Site Manager's policies and procedures. This position is responsible for maintaining positive relationships with the Funding Source, Referral Source, Community Mental Health Staff, Community Partners and Pharmacy Staff. The MDS Coordinator is responsible for entering service authorizations, completing client assessments, conducting Program Orientation Sessions- typically in the client's home, completing treatment plans, completing progress reports, progress notes, and chart notes in the electronic health record, data entry, and care coordination between the Med Drop Staff and the client's case manager/supports coordinator and treating prescriber. At times, this position will make medication drops.

Duties and Responsibilities:

· Review referrals for eligibility

· Conduct a phone assessment with the referral source

· Enter and track service authorizations

· Conduct the Program Orientation Session at the client's home or other community locations

· Complete medication reconciliation tasks

· Ensure accurate data entry into the program database

· Ensure client medical records are complete and accurate

· Ensure ALL client documentation is completed and submitted in the required time frames

· Monitor that the clients' medications are delivered at the dosage and frequency indicated by the Treating Prescriber.

· Participate in the client's treatment planning sessions as a member of the MDS Treatment Team

· Ensure the client information shared at the treatment planning sessions is accurate and current based upon conversations with the MDS Staff

· Enter monthly progress reports into EHR system. This is based upon feedback from MDS Staff and other treatment team members.

· Ensure Prescriber/MDS Treatment Team's recommendations are shared with the MDS Staff.

· Coordinate Care between client's assigned case manager/support coordinator, treating prescriber, Med Drop Pharm Tech, and MDS Staff.

· Complete discharge note and disseminate to Treating Prescriber and Case Manager.

· Gather evaluation data and accurately enter into MDS Database

· Attend all Funding Source and Genoa Pharmacy required trainings within stated time frames

· Work collaborative with MDS Provider in ensuring clients receive med drops and medication education per Genoa's Med Drop Program Policy and Procedures.

· Perform other duties as assigned

Educational or Skills Requirements:

Qualifications:

· Must have at least a bachelor degree in a human services related field - transcripts are required

· At least 1 year of experience working with adults with a mental illness

· At least 1 year of experience as a case worker is preferable, but not required

· Excellent organizational and time management skills

· Ability to work independently and with a team

· Ability to communicate clearly, concisely and professionally with the clients' health care providers

· Proficient in Microsoft Products including Word and Excel.

· Ability to type and complete accurate data entry

· Willingness to provide home/community visits
